培土生金法治疗多重耐药菌感染临床观察

Clinical Observation of Invigorating Spleen for Benefiting Lung Method in Treating Multi-drug Resistant Bacterial Infection

摘要 目的探讨中医培土生金法在多重耐药菌(MDRO)肺部感染患者中的应用效果。方法选取2020年12月—2022年12月济宁市中医院重症医学科收治的60例MDRO肺部感染患者,根据随机数字表法将其分为A组(30例)和B组(30例)。A组予以基础抗菌药物治疗,B组在A组基础上采用培土生金中药汤剂治疗,2组均持续治疗2周。观察2组中医证候积分、疗效、细菌清除效果、血清白细胞(WBC)、降钙素原(PCT)及血清乳酸(LAC)水平。结果2组治疗后各项中医证候积分均较治疗前降低,且B组分值较A组低,差异有统计学意义(P<0.05);B组治疗总有效率及总细菌清除率均较A组高,差异有统计学意义(P<0.05);较之治疗前,2组治疗后血清WBC、PCT及LAC水平更低,且B组各项指标均低于A组,差异有统计学意义(P<0.05)。结论MDRO肺部感染患者中应用培土生金法的疗效显著,有助于减轻患者症状,提高患者细菌清除率,缓解其炎症反应。 Objective To explore the application effect of invigorating spleen for benefiting lung method in treating multi-drug resistant bacteria(MDRO)pulmonary infection.Methods Sixty patients with MDRO pulmonary infection admitted to Jining Hospital of Traditional Chinese Medicine from December 2020 to December 2022 were divided into group A of 30 cases and group B of 30 cases according to the method of random number table.Group A was treated with basic antibacterial drugs,while Group B was treated with Peitu Shengjin decoction on the basis of group A,and both groups continued to be treated for 2 weeks.The TCM syndrome score,curative effect,bacterial clearance effect,serum white blood cell(WBC),procalcitonin(PCT)and serum lactic acid(LAC)levels in the two groups were observed.Results After treatment,the scores of various TCM syndromes in both groups were lower than those before treatment,and the scores in group B were lower than those in group A,and there had significant difference(P<0.05).The total effective rate and total bacterial clearance rate of group B were higher than that of group A,and there had significant difference(P<0.05).Compared with before treatment,the serum WBC,PCT and LAC levels in the two groups were lower after treatment,and the indexes in group B were lower than those in group A,and there had significant difference(P<0.05).Conclusion The invigorating spleen for benefiting lung method has a significant effect on the patients with MDRO pulmonary infection,which is helpful to alleviate the symptoms of patients,improve the bacterial clearance rate of patients,and alleviate their inflammatory reaction.
