摘要
目的观察运脾化湿清肺汤联合双歧杆菌三联活菌胶囊治疗脾虚型特应性皮炎(Atopic dermatitis,AD)患儿的疗效及对其肠道菌群的影响。方法选取2021年12月—2023年1月期间上海中医药大学附属龙华医院皮肤科门诊就诊的脾虚型AD患儿61例,采用随机数字表法分为对照组30例和治疗组31例。对照组给予运脾化湿清肺汤治疗,治疗组给予运脾化湿清肺汤联合双歧杆菌三联活菌胶囊治疗。治疗8周后,观察比较两组患儿临床疗效,治疗前后SCORAD积分及肠道菌群的变化。结果治疗后治疗组总有效率为83.87%(26/31)明显高于对照组总有效率40.00%(12/30),差异有统计学意义(P<0.01)。治疗第2周、第4周、第8周后两组患儿SCORAD积分均较治疗前降低,差异有统计学意义(P<0.05)。且治疗第4周、第8周后治疗组患儿SCORAD积分较对照组明显降低,差异有统计学意义(P<0.05)。治疗后治疗组患儿拟杆菌门(Bacteroidetes)丰度较治疗前降低,差异有统计学意义(P<0.05);且治疗组拟杆菌门(Bacteroidetes)丰度较对照组明显降低,差异有统计学意义(P<0.05)。结论运脾化湿清肺汤联合双歧杆菌三联活菌胶囊治疗脾虚型AD效果显著,并可降低肠道菌群中拟杆菌门的含量。
Objective To observe the efficacy of Yunpi Huashi Qingfei Decoction combined with Bifidobacterium Triple Viable Capsules in treating atopic dermatitis(AD)children with spleen deficiency syndrome and its impact on intestinal flora.Methods Sixty-one AD children with spleen deficiency syndrome treated in the Dermatology Department at Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine between December 2021 and January 2023 were selected and randomly divided into a control group(30 cases)and a treatment group(31 cases)using a random number table.The control group was treated with Yunpi Huashi Qingfei Decoction,while the treatment group received Yunpi Huashi Qingfei Decoction combined with Bifidobacterium Triple Viable Capsules.After 8 weeks of treatment,the clinical efficacy,SCORAD scores,and changes in intestinal flora were observed and compared between the two groups.Results After treatment,the total effective rate in the treatment group was 83.87%,significantly higher than 40.00%in the control group(P<0.01).At the 2nd,4 th,and 8th weeks of treatment,the SCORAD scores of both groups were significantly lower than those before treatment(P<0.05).Furthermore,at the 4th and 8th weeks of treatment,the SCORAD scores in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,the abundance of Bacteroidetes in the intestinal flora of the treatment group decreased significantly compared to that before treatment(P<0.05)and was also significantly lower than that in the control group(P<0.05).Conclusion Yunpi Huashi Qingfei Decoction combined with Bifidobacterium Triple Viable Capsules is highly effective in treating AD with spleen deficiency syndrome and can significantly reduce the abundance of Bacteroidetes in the intestinal flora.
