摘要
目的探讨神经营养因子联合他达拉非治疗2型糖尿病勃起功能障碍疗效及对血清神经生长因子(NGF)、脑源性神经营养因子(BDNF)、血管内皮生长因子(VEGF)表达水平的影响。方法回顾性选取2020年5月至2023年6月浙江省慈溪市龙山医院收治80例2型糖尿病勃起功能障碍患者为研究对象,按照治疗方法不同分为对照组(他达拉非治疗)40例;观察组(神经营养因子+他达拉非治疗)40例。比较2组临床疗效、血管内皮功能、血清神经营养因子、射精功能和精子质量、勃起功能障碍指数和勃起硬度以及性生活质量。结果治疗后观察组临床总有效高于对照组(χ^(2)=4.581、P=0.032);治疗8周后观察组血清内皮素(ET-1)低于对照组,6酮前列腺素F1a(6-keto-PGF1a)、前列环素(PGI2)、VEGF高于对照组(t=8.057、P<0.01,t=4.688、P<0.01,t=2.538、P=0.013,t=3.969、P<0.01);观察组NGF、BDNF、胶质源性神经营养因子(GDNF)水平高于对照组(t=2.094、P=0.039,t=2.124、P=0.037,t=2.876、P=0.005);观察组射精功能量表(CI-PE)评分高于对照组,精子数量、精子密度、精子活力大于对照组(t=2.459、P=0.016,t=2.821、P=0.006,t=3.370、P=0.001,t=4.181、P<0.01);观察组勃起功能指数(IIEF-5)、勃起硬度分级(EHGS)、性生活质量量表评分高于对照组(t=6.242、P<0.01,t=2.658、P=0.009,t=2.340、P=0.022)。结论神经营养因子联合他达拉非治疗2型糖尿病勃起功能障碍临床疗效显著,可有效提高血管内皮功能和血清神经营养因子水平,提高射精和勃起功能,以及提升性生活质量,安全性良好。
Objective To investigate the therapeutic effect of neurotrophic factor combined with tadalafil on erectile dysfunction in type 2 diabetes and its effect on the expression of NGF,BDNF and VEGF.Methods A retrospective study was conducted on 80 patients with type 2 diabetes mellitus and erectile dysfunction admitted to Longshan Hospital in Cixi City from May 2020 to June 2023.They were divided into a control group(treated with tadalafil,40 cases)and an observation group(treated with neurotrophic factors+tadalafil,40 cases)based on different treatment methods.The clinical efficacy,vascular endothelial function,serum neurotrophic factors,ejaculation function and sperm quality,erectile dysfunction index,erectile hardness,and sexual quality of life were compared between the two groups.Results After treatment,the clinical response rate in the observation group was higher than that in the control group(χ^(2)=4.581,P=0.032).After 8 weeks of treatment,the serum endothelin-1(ET-1)level in the observation group was lower than that in the control group,while 6-keto-prostaglandin F1a(6-keto-PGF1A),prostacyclin(PGI2),and VEGF were higher than those in the control group(t=8.057,P<0.01;t=4.688,P<0.01;t=2.538,P=0.013;t=3.969,P<0.01).The levels of nerve growth factor(NGF),brain-derived neurotrophic factor(BDNF),and glial cell line-derived neurotrophic factor(GDNF)in the observation group were higher than those in the control group(t=2.094,P=0.039;t=2.124,P=0.037;t=2.876,P=0.005).The ejaculatory function scale(CI-PE)score in the observation group was higher than that in the control group,and the sperm count,sperm density,and sperm motility were all greater than those in the control group(t=2.459,P=0.016;t=2.821,P=0.006;t=3.370,P=0.001;t=4.181,P<0.01).The erectile function index(IIEF-5),erectile hardness grading scale(EHGS),and sexual quality of life scale scores in the observation group were higher than those in the control group(t=6.242,P<0.01;t=2.658,P=0.009;t=2.340,P=0.022).Conclusion The combined efficacy of neurotrophic factors
