摘要
目的 探索形成性评价在临床病理科住院医师标本取材中的意义和效果。方法 自制《住院医师取材质量形成性评价表》,评分标准包括大体描述、材块切取和记录和大体照相三个指标,根据正确病例比率分为优(比率≥90%)、良(比率≥75%)、中(比率≥60%)和差(比率<60%)四个档次。同时指导医师及时对住院医师进行反馈,住院医师根据反馈意见整改。每月评估1次,连续评估6个月,收集数据并分析总结。结果 6个月共收集42份住院医师取材质量形成性评价表,评估807例标本。住院医师取材质量形成性评价表中完全正确率有逐渐升高的趋势,基本正确率有逐渐下降的趋势,错误仅为个别现象,错误率有逐渐下降趋势。取材质量优31份,良8份,中2份,差1份。每月补取率2%左右。结论 形成性评价可以对住院医师的标本取材质量进行量化,发现存在的问题并改进,能够逐步提高其取材水平和质量,从而增强培训效果。
Objective To explore the significance and effectiveness of formative assessment in the specimen collection process for clinical pathology residents.MethodsWe proposed a specifically designed formative evaluation form which was used to evaluate the specimen collection performance of clinical pathological residents.And based on the accuracy rate on following three aspects:gross description of the specimen,samples cutting and collection,as well as samples photography.The performance of the residents would be classified into four levels:excellent(accuracy rate≥90%),good(accuracy rate≥75%),medium(accuracy rate≥60%),and poor(accuracy rate<60%).The instructor of each resident would give the performance result back to each individual,and the residents shall be granted the chance to learn from the feedback,thus keeping improving.The evaluation would be performed at least once a month for continuous six months,followed by the analysis and summary of the final data.Results A total of 42 formative evaluation forms containing 807 specimens were collected during the 6 month period.And based on the data,an inspiring result was discovered that the accuracy rate kept increasing,meanwhile,the errors as well as partly error rate decreased over the time.Actually,at the final stage of evaluation,errors only occurred occasionally.Of the 42 performance forms,a total of 31 were evaluated as excellent,8 were good,2 were media,meanwhile,only 1 was evaluated as poor.The replenishment rate was estimated to be around 2%each month.Conclusion Our specifically designed formative evaluation form could be effectively used to evaluate the performance of training residents,thus aiding the identifying of existing problems followed by being fixed.The formative evaluation form is expected to be able to gradually improve the students'specimen collection ability,and we consider it suitable for future wide-ranging promotion and clinical application during the process of students training.
