摘要
稳定型心绞痛是冠心病最常见的临床类型。冠心病二级预防作为其最主要防治策略,在长期的降脂稳斑及抗血小板治疗中可出现药物性肝肾损伤等多种不良反应,使患者依从性变差,冠心病病情加重,甚至进展为不稳定型心绞痛等急性心血管事件,影响患者生活质量和远期预后。针刺、艾灸、穴位贴敷、中药热奄包、刮痧、拔罐、健身气功等多种中医适宜技术辅助治疗稳定型心绞痛疗效明确,可多环节干预冠心病心绞痛的发生发展,但由于缺乏中医辨证理论指导和操作方法的详细阐释,使其治疗的针对性不强,受益人群有限。文章系统梳理稳定型心绞痛常用中医适宜技术的作用机制、作用特点、适宜人群、操作要点、注意事项、禁忌证,使其更加规范化、精准化地应用于稳定型心绞痛治疗,提高临床疗效,有着非要重要的现实意义。
Stable angina is the most common clinical type of coronary heart disease.Secondary prevention of coronary heart disease,as the main prevention and treatment strategy,may lead to drug induced liver and kidney injury and other adverse reactions in long-term lipid-lowering and plaque stabilization and antiplatelet therapy,which will lead to poor compliance,aggravation of coronary heart disease,and even progression to acute cardiovascular events such as unstable angina,affecting the quality of life and long-term prognosis of patients.It is found that acupuncture,moxibustion,acupoint application,traditional Chinese medicine(TCM) hot package,scraping,cupping,fitness Qigong and other appropriate techniques of TCM are effective in assisting the treatment of stable angina pectoris,and the occurrence and development of coronary heart disease angina pectoris could be intervened through multiple targets and links.However,due to the lack of guidance of TCM syndrome differentiation theory and detailed explanation of operation methods,its treatment is not targeted and the beneficiary population is limited.It is of great practical significance to systematically sort out the mechanism of action,action characteristics,suitable people,operating points,precautions and contraindications of the commonly used TCM appropriate techniques for stable angina pectoris,so as to make it more standardized and accurately applied to the treatment of stable angina pectoris and improve the clinical efficacy.
