摘要
目的 系统评价阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与卵圆孔未闭(PFO)之间的相关性。方法 检索PubMed、EMbase、Wed of Science、Cochrane Library、中国知网、万方、CBM和维普等数据库中OSAHS与PFO的病例对照研究,检索时限均从建库至2023年5月。对符合条件的文献,由2位评价员独立进行文献筛查、提取数据,偏倚风险质量评估后采用RevMan5.4.1软件进行meta分析。结果 有6项研究符合纳入标准,共924例患者。其中,6项研究显示OSAHS与PFO存在相关性(RR=2.22,95%CI 1.76~2.79,P<0.001)。按照患者不同状态下进行PFO检查的亚组分析结果显示,在休息状态下患者不易检出PFO的发生(RR=1.98,95%CI 0.95~4.13,P=0.07);而在Valsalva状态下患者易检出PFO的发生(RR=2.20,95%CI 1.74~2.78,P<0.001)。患者做Valsalva动作时更容易引起患者心房内由右向左分流,提高PFO的检出率。结论 OSAHS与PFO呈明显的相关性,OSAHS患者中PFO的发生率较高。
Objective To systematically evaluate the correlation between obstructive sleep apnea hypopnea(OSAHS)and patent foramen ovale(PFO).Methods A computerized search of PubMed,EMbase,Wed of Science,Cochrane Library,China National Knowledge Infrastructure,Wanfang Database,CBM and VIP was applied to retrieve case-control studies of OSAHS and PFO,with the timeframe of searching from the establishment of the library to May 2023 in all cases.For eligible literature,two evaluators independently performed literature screening,extracted data,and performed meta-analysis using RevMan 5.4.1 software after quality assessment of risk of bias.Results A total of six studies met the inclusion criteria,totaling 924 patients.Among them,six studies showed a correlation between OSAHS and PFO(RR=2.22,95%CI 1.76-2.79,P<0.001).Subgroup analysis of PFO examination according to different states of the patients showed that the occurrence of PFO was not easily detected in patients in the resting state(RR=1.98,95%CI 0.95-4.13,P=0.07),while the occurrence of PFO was easily detected in patients in the Valsalva state(RR=2.20,95%CI 1.74-2.78,P<0.001).The patients performing Valsalva maneuvers were more likely to cause right-to-left shunting in the patient′s atria,increasing the detection rate of PFO.Conclusion Current evidence suggests that OSAHS shows a clear correlation with PFO and the incidence of PFO is higher in patients with OSAHS.
