超声心动图定量评估心肌做功参数与非梗阻型肥厚型心肌病左室收缩功能的关系探究

Quantitative evaluation of the relationship between myocardial work parameters and left ventricular systolic function in non-obstructive hypertrophic cardiomyopathy by echocardiography

摘要 目的探讨心肌做功参数和非梗阻型肥厚型心肌病(HCM)患者左室收缩功能的关系。方法选取2021年3月至2023年4月我院收治的81例非梗阻型HCM患者作为研究组,另选取同期90例健康体检者作为对照组,均行超声心动图检查。比较两组的左室收缩功能指标与心肌做功参数,并分析两者的相关性。结果两组的左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)及左室射血分数(LVEF)无明显差异(P>0.05);研究组的室间隔厚度(IVST)、左房前后径(LAD)及整体纵向应变(GLS)大于对照组,E峰/A峰小于对照组,差异具有统计学意义(P<0.05)。研究组的整体做功指数(GWI)、整体有用功(GCW)及整体做功效率(GWE)均低于对照组,差异具有统计学意义(P<0.05);两组的整体无用功(GWW)无明显差异(P>0.05)。Pearson相关性分析结果表明,GWI、GWW、GCW、GWE与LVEDD、LVESD、LVEF无明确关联性(P>0.05);GWI、GCW、GWE均与IVST、LAD、GLS呈负相关,与E峰/A峰呈正相关(P<0.05)。结论与左室收缩功能指标相比,心肌做功参数更有助于诊断非梗阻型HCM。 Objective To investigate the relationship between myocardial work parameters and left ventricular systolic function in patients with non-obstructive hypertrophic cardiomyopathy(HCM).Methods A total of 81 patients with non-obstructive HCM admitted in our hospital from March 2021 to April 2023 were selected as the study group,and 90 healthy subjects who underwent physical examinations during the same period were selected as the control group.All patients underwent echocardiography.The left ventricular systolic function indexes and myocardial work parameters of the two groups were compared,and the correlation between the two was analyzed.Results There were no significant differences in left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)and left ventricular eiection fraction(LVEF)between the two groups(P>0.05);the inter-ventricular septum thickness(IVST),left atrium anteroposterior diameter(LAD)and global longitudinal strain(GLS)in the study group were greater than those in the control group,and the E peak/A peak was less than that in the control group,the differences were statistally significant(P<0.05).The global work index(GWI),global constructive work(GCW)and global work efficiency(GWE)of the study group were lower than those of the control group,and the differences were statistally significant(P<0.05);there was no significant difference in global waste work(GWW)between the two groups(P>0.05).Pearson correlation analysis showed that there were no clear correlation between GWI,GWW,GCW,GWE and LVEDD,LVESD,LVEF(P>0.05);GWI,GCW and GWE were negatively correlated with IVST,LAD and GLS,and positively correlated with E peak/A peak(P<0.05).Conclusion Compared with left ventricular systolic function parameters,myocardial work parameters are more helpful in the diagnosis of non-obstructive HCM.
