摘要
目的:分析腹腔镜胆囊切除术对慢性胆囊炎合并肝功能不全患者应激反应及胃肠功能的影响。方法:选取2020年8月~2023年8月本院收治的130例慢性胆囊炎患者作为研究对象,根据是否合并肝功能不全将患者分为观察组(n=60)和对照组(n=70)。对比两组在应激反应、胃肠功能、手术指标、住院时间和术后并发症差异。结果:两组术中出血量、手术时间、中转开腹率和住院时间无明显差异(P>0.05);术后3 d两组超敏C反应蛋白和外周血白细胞计数水平均明显升高(P<0.05),但两组无明显差异(P>0.05);两组肠鸣音恢复时间、肛门排气时间、首次排便时间无明显差异(P>0.05);两组术后并发症发生率无明显差异(P>0.05)。结论:腹腔镜胆囊切除术对于慢性胆囊炎合并肝功能不全患者会提高其应激水平,但对患者的影响较小,与单纯慢性胆囊炎患者比较无明显差异,可采取腹腔镜下胆囊切除术治疗,帮助患者尽早康复。
Objective:To analyze the effects of laparoscopic cholecystectomy on stress response and gastrointestinal function in patients with chronic cholecystitis and hepatic insufficiency.Methods:A total of 130 patients with chronic cholecystitis admitted to the hospital from August 2020 to August 2023 were selected as the study subjects.According to the presence and absence of hepatic insufficiency,the patients were divided into observation group(n=60)and control group(n=70).Stress response,gastrointestinal function,surgical indicators,length of hospital stay and postoperative complications were compared between the two groups.Results:There was no significant difference in intraoperative blood loss,surgical time,rate of conversion to open surgery,or length of hospital stay between the two groups(P>0.05).On day 3 after surgery,the levels of high-sensitivity C-reactive protein and peripheral blood leukocyte count in both groups increased significantly(P<0.05),but there was no significant difference between the groups(P>0.05).There was no significant difference in recovery time for bowel sounds,anal exhaust time,or time to first bowel movement between the groups(P>0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:Laparoscopic cholecystectomy can increase the stress level of patients with chronic cholecystitis and hepatic insufficiency,but there is little influence on patients.Compared to patients with simple chronic cholecystitis,there is no significant difference.Laparoscopic cholecystectomy can be adopted to help patients recover as soon as possible.
