海洛因渴求度评估的血清学生物标志物研究

The study on serological biomarkers for the measurement of heroin craving

摘要 目的海洛因成瘾是一种慢性、复发性脑病,具有高度的精神和躯体依赖性,氧化应激、炎症反应、神经因子代谢紊乱被证明参与其成瘾的病理生理机制。开发海洛因成瘾的生物标志物对于其易感性预测、临床干预及治疗效果的评估至关重要,因此,本文力求筛选并探讨潜在性成瘾机制关联因子丙二醛(MDA)、肿瘤坏死因子α(TNF-α)、脑源性神经营养因子(BDNF)及其载体血小板作为海洛因成瘾性血清学生物标志物的可行性和实用性。方法以正常人群52名和海洛因依赖人群51名分别作为对照组和实验组。采用全自动血细胞分析仪测定其血小板数量、酶联免疫吸附法(Enzyme-linked Immunosorbent Assay,ELISA)检测各组别血清MDA、TNF-α及血浆BDNF的含量,采用海洛因渴求评分量表对实验组个体的成瘾性进行评估、Spearman相关性分析评估血清MDA、TNF-α、血浆BDNF和血小板数量与渴求评分的相关性、受试者工作特征曲线(Receiver Operating Characteristic Curve,ROC)评估血浆BDNF和血小板数量对海洛因成瘾性评分的诊断性能及截断值。结果实验组人群血清MDA、TNF-α高于对照组(P<0.05),血浆BDNF和血小板数量低于对照组(P<0.01)。海洛因高成瘾性组血浆BDNF及血小板数量低于海洛因低成瘾性组(P<0.05),实验组血浆BDNF、血小板数量和成瘾性评分呈负相关。血小板数量对海洛因成瘾性的灵敏度和特异度为69%和12%,AUC值为0.76(95%CI:0.66-0.87,P<0.001)。结论研究初步表明,长期海洛因摄入可显著下调血浆BDNF水平和血小板数量,血小板数量可作为评估海洛因成瘾性的新型血清学生物标志物。 Objective Heroin addiction is a chronically relapsing encephalopathy with a severe mental and physical dependency.Oxidative stress,inflammatory response and metabolic disorders of neurotrophic factors have been involved in the pathophysiology of heroin addiction.The development of biomarkers for heroin addiction is essential for the prediction of vulnerability,clinical course,and response to treatment.Therefore,this paper aims to screen and explore the feasibility and practicality of the potential addiction mechanism-related factors malonaldehyde(MDA),tumor necrosis factorα(TNF-α),brain-derived neurotrophic factor(BDNF)and its carrier platelets acting as serological biomarkers for heroin addiction.Methods 52 healthy individuals and 51 heroin-dependent patients from a certain drug rehabilitation center were arranged as control group and experimental group respectively.The platelet counts were measured with automatic blood cell analyzer.Serum MDA,TNF-αand plasma BDNF were detected by enzyme-linked immunosorbent assay(ELISA)and the addiction degree was evaluated with the heroin craving score for the experimental group individuals.Furthermore,the correlation between serum MDA,TNF-α,plasma BDNF and craving scores were measured by spearman correlation analysis.Finally,the diagnostic performance and cut-off value of plasma BDNF and platelet count for heroin addiction were analysed with receiver operating characteristic curve(ROC).Results Compared with control group,serum MDA and TNF-αwere higher(P<0.05)while plasma BDNF and platelet counts were lower in the experimental group(P<0.01).Plasma BDNF and platelet counts in the heroin high-addiction group were lower than those in the heroin low-addiction group(P<0.05).In addition,Plasma BDNF levels and platelet counts showed a negative association with the craving score.ROC curve analysis results showed that the sensitivity and specificity of platelet count to heroin addiction were 69%and 12%,the AUC value was 0.76(95%CI:0.66-0.87,P<0.001).Conclusion Long-term heroin int
