摘要
目的:探讨精神分裂症患者使用越鞠丸联合奥氮平治疗的疗效及其可能机制。方法:按照治疗方式不同将97例精神分裂症患者分为试验组(n=48)与对照组(n=49)。两组均予以奥氮平治疗,试验组予以越鞠丸口服,对照组予以越鞠丸安慰剂口服,两组均连续治疗8周。统计两组患者疗效及不良反应发生情况,于治疗前后使用阳性与阴性症状量表(PANSS)、阴性症状量表(SANS)对患者进行精神症状评定,使用可重复成套神经心理状态测量(RBANS)对患者认知功能进行评定,使用个人和社会功能量表(PSP)对患者社会功能进行评定,测定并比较两组患者血清脑源性神经营养因子(BDNF)、多巴胺(DA)、5-羟色胺(5-HT)水平。结果:试验组患者治疗总有效率为93.75%,高于对照组的73.47%(P<0.05)。治疗后,试验组PANSS阴性因子、PANSS总分及SANS评分均低于对照组(P<0.05);RBANS评分、PSP评分均高于对照组(P<0.05)。治疗后,试验组患者血清BDNF、DA、5-HT水平均高于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:越鞠丸联合奥氮平能够改善精神分裂症患者阴性症状和认知功能,作用机制可能与上调血清BDNF、DA、5-HT表达有关。
Objective:To investigate the efficacy and mechanism of Yueju Pills combined with olanzapine in the treatment of schizophrenia.Methods:97 patients with schizophrenia were divided into experimental group(n=48)and control group(n=49)according to the different treatment methods.All patients were treated with olanzapine.In addition,the experimental group was given Yueju pill orally,and the control group was given Yueju pill placebo orally.Both groups were treated continuously for 8 weeks.The efficacy and adverse reactions of the two groups were statistically analyzed.Before and after treatment,the positive and negative symptom scale(PANSS),negative symptom scale(SANS),repeatable battery of neuropsychological status measurement(RBANS),personal and social function scale(PSP)were evaluated.The serum levels of Brain-derived neurotrophic factor(BDNF),dopamine(DA)and 5-hydroxytryptamine(5-HT)were measured and compared between the two groups.Results:The total effective rate of the experimental group was 93.75%,which was higher than 73.47%of the control group(P<0.05).After treatment,the PANSS negative factor,PANSS total score and SANS score in the experimental group were lower than those in the control group(P<0.05),and the RBANS score and PSP score were higher than those in the control group(P<0.05).After treatment,the serum BDNF,DA and 5-HT levels in the experimental group were higher than those in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Yueju pill combined with olanzapine can improve the negative symptoms and cognitive function of schizophrenic patients,its mechanism may be related to the up-regulation of serum BDNF,DA and 5-HT expression.
