3D-STI评估2型糖尿病视网膜病变患者左室心肌应变的应用

摘要 目的:应用三维斑点追踪成像(three-dimensional speckle tracking imaging,3D-STI)技术评估射血分数保留的2型糖尿病(diabetes mellitus type 2,T2DM)合并糖尿病视网膜病变(diabetic retinopathy,DR)患者的左室心肌损伤情况,探讨3D-STI技术评价此类患者左室收缩功能的临床应用价值。方法:选取2023年2月至2024年2月佳木斯大学附属第一医院内分泌科住院收治的射血分数保留的T2DM患者共50例作为病例组,根据眼底照相结果将患者分为DR组25例和非DR组25例,同期体检的健康志愿者25例作为对照组。采集左室全容积图像,应用3D-STI技术获取射血分数(3D-EF)、整体纵向应变(GLPS)、圆周应变(GCPS)、面积应变(GAPS),并进行比较分析。结果:对照组与非DR组、DR组3D-LVEF、GAPS、GLPS、GCPS指标有统计学差异(P<0.05);进一步两两比较结果显示,非DR组和DR组3D-LVEF、GAPS、GLPS、GCPS绝对值均低于对照组,DR组GLPS绝对值低于非DR组,有统计学差异(P<0.05)。3D-LVEF、GLPS、GCPS、GAPS绝对值与疾病复杂程度均呈现负相关关系。结论:3D-STI能够早于常规超声心动图发现射血分数保留的T2DM患者左室收缩功能的变化;T2DM合并DR者左室心肌损伤更为明显。 Objective:To evaluate the left ventricular myocardial strain in patients with type 2 diabetes mellitus(T2DM)and diabetic retinopathy(DR)with preserved ejection fraction by three-dimensional speckle tracking imaging(3D-STI)technique,and to explore the clinical application value of 3D-STI in evaluating left ventricular systolic function in such patients.Methods:From February 2023 to February 2024,a total of 50 T2DM patients with preserved ejection fraction were selected as the case group.According to the results of fundus photography,the patients were divided into DR group(25 cases)and non-DR group(25 cases),and 25 healthy volunteers who came to our hospital for physical examination at the same time served as the control group.Left ventricular full volume images were collected,and ejection fraction(3D-EF),global longitudinal peak strain(GLPS),global circumferential peak strain(GCPS)and global area peak strain(GAPS)were obtained by 3D-STI technology,and compared and analyzed.Results:The indexes of 3D-LVEF,GAPS,GLPS and GCPS in control group were statistically different from those in non-DR group and DR group(P<0.05).The results of further pairwise comparison showed that the absolute values of 3D-LVEF,GAPS,GLPS and GCPS in non-DR group and DR group were lower than those in control group,and the absolute values of GLPS in DR group were lower than those in non-DR group,with statistical difference(P<0.05).The absolute values of 3D-LVEF,GLPS,GCPS and GAPS were negatively correlated with the complexity of the disease.Conclusion:3D-STI can detect the changes of left ventricular systolic function in T2DM patients with preserved ejection fraction earlier than conventional echocardiography.In T2DM patients with DR,the left ventricular myocardial injury is more obvious.
