摘要
目的探讨念珠菌性阴道炎患者白色念珠菌烯醇化酶含量与阴道微生态、炎性因子水平及疗效的关系。方法收集2018年1月至2019年12月于中国人民解放军联勤保障部队第九八○医院进行治疗的360例念珠菌性阴道炎患者作为研究对象,根据疗效分为有效组(n=292)和无效组(n=68)。比较两组白色念珠菌烯醇化酶含量、阴道微生态指标、炎性因子水平;绘制受试者工作特征(ROC)曲线分析白色念珠菌烯醇化酶含量对念珠菌性阴道炎患者疗效的预测价值;采用Spearman等级相关分析念珠菌性阴道炎患者白色念珠菌烯醇化酶含量与阴道微生态指标之间的相关性;采用Pearson法分析念珠菌性阴道炎患者白色念珠菌烯醇化酶含量与炎性因子水平的相关性。结果无效组患者白色念珠菌烯醇化酶含量、阴道清洁度为Ⅲ~Ⅳ度所占比例以及炎性因子[C反应蛋白(CRP)、降钙素原(PCT)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平高于有效组(P<0.05),阴道微生态指标(pH值≤4.5、乳酸杆菌大量所占比例)低于有效组(P<0.05)。白色念珠菌烯醇化酶含量预测念珠菌性阴道炎患者治疗无效的曲线下面积(AUC)为0.775(95%CI:0.728~0.817),最佳截断值为28.63 ng/mL,特异度为86.30%,灵敏度为63.24%。Spearman相关性分析结果显示,白色念珠菌烯醇化酶含量与阴道微生态指标(pH值、乳酸杆菌、阴道清洁度)均具有相关性(P<0.05)。Pearson法分析显示,白色念珠菌烯醇化酶含量与CRP(r=0.514,P<0.001)、PCT(r=0.482,P<0.001)、IL-6(r=0.578,P<0.001)、TNF-α(r=0.231,P=0.018)呈正相关。结论念珠菌性阴道炎患者白色念珠菌烯醇化酶含量升高,且与患者阴道微生态、炎性因子及疗效密切相关。
Objective To explore the relationship between the content of Candida albicans enolase and vaginal microbiota,inflammatory factors,and efficacy in patients with candidal vaginitis.Methods A total of 360 patients with candidal vaginitis treated in No.980 Hospital of Chinese PLA from January 2018 to December 2019 were collected as research subjects,and were grouped into an effective group(n=292)and an ineffective group(n=68)based on their efficacy.The levels of Candida albicans enolase,vaginal microbiota,and inflammatory factors in two groups were compared.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of Candida albicans enolase content on the efficacy of candidal vaginitis patients.Spearman rank correlation was applied to analyze the correlation between the contents of Candida albicans enolase and vaginal microbiota indicators in patients with candidal vaginitis.Pearson method was applied to analyze the correlation between the content of Candida albicans enolase and inflammatory factors levels in patients with candidal vaginitis.Results The content of Candida albicans enolase,the proportion of vaginal cleanliness of gradeⅢ-Ⅳ,and inflammatory factors such as C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)in the ineffective group higher than those in the effective group(P<0.05),the proportions of the vaginal microbiota index pH value≤4.5,and large amount of lactic acid bacteria were lower than those in the effective group(P<0.05).The area under the curve(AUC)of Candida albicans enolase in predicting ineffective treatment in patients with candidal vaginitis was 0.775(95%CI:0.728-0.817),with an optimal cutoff value of 28.63 ng/mL,specificity of 86.30%,and sensitivity of 63.24%.The Spearman correlation analysis results showed that the content of Candida albicans enolase was correlated with the vaginal microecological indicators-pH value,lactobacilli,and vaginal cleanliness(P<0.05).Pearson analysis showed that the content
