摘要
目的本文对颈椎病的运动疗法及机制进展进行综述,以期进一步为本病临床运动处方的制定和相关研究的开展提供参考。方法基于文献研究,以“颈椎病”“颈痛”“运动治疗”“运动控制”“悬吊”“传统功法”“八段锦”等为关键词,在中国知网、万方知识服务平台、维普网数据库中检索中文文献;以“cervical spondylosis”“neck pain”“exercise therapy”“motor control training”“sling exercise training”“qigong”“Baduanjin”为关键词,在PubMed、Web of Science数据库中检索英文文献,检索时间范围2013年2月至2023年5月。结果在治疗颈椎病的运动疗法中,传统功法如八段锦、太极拳、五禽戏等运动强度小,简便易学,安全有效;现代运动疗法如运动控制、悬吊、牵伸训练等运动强度不等、坚持治疗疗效显著;运动疗法联合其他常规治疗或是运动疗法间的联合治疗效果更佳。运动疗法通过拉伸颈肌促进血液循环,激活脊髓上中枢镇痛通路,增加抗炎信号转导,降低肿瘤坏死因子-α(TNF-α)水平,发挥镇痛作用;增强肌力,改善僵硬和疲劳,恢复椎体力学平衡;增加神经与肌肉间的反馈,改善颈部本体感觉。结论运动疗法主要通过抗炎镇痛、改善颈椎感觉与运动功能来治疗颈椎病。运动疗法可有效改善颈椎病患者颈部疼痛和功能障碍,缓解患者焦虑不安等不良情绪,提高患者生活质量。
Objective To review the research progress of exercise therapy of cervical spondylosis and its mechanism,thus providing references for the formulation of clinical exercise prescription and related research.Methods Based on literature research,Chinese-language literatures published from February 2013 to May 2023 were searched in China National Knowledge Infrastructure(CNKI),Wanfang Data and VIP database using the key words of"cervical spondylosis""neck pain""exercise therapy""exercise control""suspension""traditional exercises"and"Baduanjin".English-language literatures published in the same period were searched in PubMed and Web of Science using the key words of"cervical spondylosis""neck pain""exercise therapy""motor control training""sling exercise training""qigong"and"Baduanjin".Results Among the treatments of cervical spondylosis,traditional exercises such as Baduanjin,Taijiquan,Wuqin play were characterized by small exercise intensity,easy to learn,safety and effectiveness.Modern exercise therapies such as motor control,sling exercise training,stretch training and others had varying exercise intensity,and remarkable efficacy after a persistence treatment.Exercise therapy was more effective in combination with other conventional treatments or multiple types of exercise therapy.Exercise therapy was able to promote blood circulation by stretching the neck muscle,activating the central analgesic pathway in the spinal cord,increasing anti-inflammatory signal transduction,reducing the level of tumor necrosis factor-α(TNF-α),and thus played an analgesic role.It also enhanced muscle strength,improved stiffness and fatigue,restored vertebral physical balance.Exercise therapy increased the feedback between nerve and muscle,and improved neck proprioception.Conclusion Exercise therapy treats cervical spondylosis through anti-inflammatory analgesia,improvement of cervical sensation and motor function.It effectively alleviates the neck pain and dysfunction of patients with cervical spondylosis,relieves anxiety and other
