摘要
目的 探究MRI增强前T_(1) mapping成像联合扩散加权成像在乳腺病变应用中的诊断价值。方法 前瞻性连续搜集44例女性乳腺病变患者的MRI图像,由两名影像科医师独立勾画每个病灶的感兴趣区并测量T_(1)值和表观扩散系数(ADC)值。使用组内相关系数评估观察者间的一致性,分别评估并比较T_(1)值、ADC值和T_(1)+ADC值对于乳腺病变良恶性鉴别的诊断效能,计算受试者工作特征曲线下面积(AUC)、特异度、敏感度。使用Spearman分析T_(1)值与乳腺癌病理特征的相关性。结果 44例患者共50个病灶(17个良性病灶,33个恶性病灶)。良性组和恶性组之间的ADC值和T_(1)值差异具有统计学意义(P<0.001),T_(1)+ADC值具有最优的鉴别诊断效能,AUC为0.989。T_(1)值与腋窝淋巴结转移状态呈负相关且差异存在统计学意义(r=-0.472,P<0.01)。结论 增强前T_(1) mapping成像有潜力鉴别乳腺良恶性病变,T_(1)值联合ADC值能进一步提高诊断准确性。
Objective This study aimed to explore the diagnostic value of pre⁃contrast T_(1) mapping combined with diffu⁃sion⁃weighted imaging(DWI)in magnetic resonance imaging(MRI)for breast lesion assessment.Methods A prospec⁃tive continuous collection of MR images was conducted on 44 female patients with breast lesions.Two radiologists independ⁃ently delineated regions of interest for each lesion and analyzed T_(1) values and apparent diffusion coefficient(ADC)values.Intra⁃class correlation coefficients(ICC)were used to assess inter⁃observer consistency.Comparative evaluations were per⁃formed for three assessment methods(T_(1) values,ADC values,and T_(1)+ADC values)regarding their diagnostic efficacy in distinguishing between benign and malignant breast lesions.Receiver operating characteristic(ROC)curves,area under the curve(AUC),specificity,and sensitivity were calculated.Spearman analysis was employed to assess the correlation between T_(1) values and breast cancer pathological features.Results A total of 44 patients with 50 lesions(17 benign,33 malig⁃nant)were included in the breast MRI analysis.Significant differences in ADC values and T_(1) values were observed between the benign and malignant groups(P<0.001).T_(1)+ADC values demonstrated optimal diagnostic efficacy with an AUC of 0.989.T_(1) values showed a significant negative correlation and statistical difference with axillary lymph node metastasis sta⁃tus(r=-0.472,P<0.01).Conclusion Pre⁃contrast T_(1) mapping have the potential to differentiate between benign and malignant breast lesions,and the combined use of T_(1) values and ADC values further enhances diagnostic accuracy.
