摘要
目的了解天津市≥60岁社区老年人跌倒发生情况,探索影响老年人跌倒的可能危险因素,为开展预防老年人跌倒工作及相关政策制定提供参考依据。方法采用分层随机抽样的方式,选择天津市内6区≥60岁社区老年人并按照男女比例1∶1进行随机抽样,于2021年9—12月对抽样人群进行问卷调查以收集数据,并对数据进行分析。结果共收集610份有效问卷。调查人群在1年内发生过跌倒的人数为95人,跌倒发生率为15.6%,男性(16.7%)略高于女性(14.4%)。多因素logistic回归分析结果显示,子女到访频率低于1次/月(OR=1.94,95%CI:1.11~3.38)、健康状况自评较差(OR=5.70,95%CI:1.94~16.74)和患有高血压(OR=1.77,95%CI:1.04~3.01)的老年人跌倒风险更高,而年龄在70~75岁之间的老年人(OR=0.29,95%CI:0.15~0.56)其跌倒风险显著低于其他年龄段。结论天津市老年人跌倒发生率相对较高,特定年龄段、具有高血压、子女来访频率较低及健康状况自评较差是跌倒发生的高危因素。
Objective To investigate the prevalence of falls among the elderly aged 60 years and above in the community of Tianjin City and to explore the possible risk factors affecting the occurrence of falls among the elderly,and to provide a theoretical basis for the prevention of falls among the elderly and the formulation of related policies.Methods A stratified random sample was used to select the target population from six districts in Tianjin City with a male to female ratio of 1∶1,a questionnaire survey was conducted to collect data from the sample population during September to December 2021,and the data were analyzed.Results A total of 610 valid questionnaires were collected.The number of people who had a fall within a year in the survey population was 95,with a fall incidence of 15.6%,and a higher incidence of falls in male(16.7%)than in female(14.4%).Multiple logistic regression analysis showed that low frequency of child visits(OR=1.94,95%CI:1.11-3.38),poor self-rated health status(OR=5.70,95%CI:1.94-16.74)and having hypertension(OR=1.77,95%CI:1.04-3.01)were all associated with a higher risk of falls,while elderly in the 70-<75 age group(OR=0.29,95%CI:0.15-0.56)had a significantly lower risk of falls than other age groups.Conclusion The incidence of falls among the elderly in Tianjin City is relatively high.Specific age groups,having high blood pressure,less frequent visits from children and poor self-assessment of health are high risk factors for the occurrence of falls.
