摘要
目的应用锥形束CT(CBCT)扫描法研究846颗青岛地区上颌第一磨牙的解剖特点。方法随机选取青岛大学附属青岛市口腔医院放射科拍摄CBCT的成人数据资料中465例患者的846颗上颌第一磨牙,根据年龄分组,分析其牙根数目、根管数目、根管形态、髓室高度、根尖与上颌窦底关系的解剖特点。结果①牙根数目:上颌第一磨牙牙根以三根为主。②根管数目:在18~49岁组中以四根管为主,发现2例五根管病例出现在30~39岁年龄组。在50~69岁组中以三根管为主。③根管形态:在18~49岁组中MB根管与MB2根管多为I型,在50~69岁组MB根管多为I型,各年龄组DB根管均为Ⅰ型,P根管以腭侧单根单根管为主。④髓腔高度:随年龄增加髓腔高度呈现逐渐降低的趋势。⑤根尖与上颌窦底关系:各牙根根尖与上颌窦底之间的关系以平齐为主。结论青岛地区上颌第一磨牙根管系统复杂多变,并且随年龄增长根管数目、根管形态、髓腔高度及根尖与上颌窦底关系也有不同表现,CBCT扫描法可有效发现各年龄段各型根管解剖特点,对临床治疗具有重要的意义,为临床诊疗提供数据支持。
Objective To analyze the anatomical characteristics of 846 maxillary first molars in subjects from Qingdao municipality of China by cone beam computed tomography(CBCT).Methods sClinical data of 846 cases of adult maxillary first molars(465 patients)examined by CBCT in the Department of Radiology,Qingdao Stomatological Hospital Affiliated to Qingdao University were randomly collected.Stratified by the age,anatomical characteristics of the number of roots,number of root canals,morphology of root canals,height of pulp chambers,and the anatomical relationship between the root tip and the maxillary sinus floor were analyzed.Results Three roots are detected in most of maxillary first molars.Four root canals are mainly detected in the maxillary first molars of subjects with the age of 18-49 years.Five root canals are detected in 2 subjects with the age of 30-39 years.Three root canals are mainly observed in subjects aged 50-69.Most of the second mesiobuccal(MB2)root canals of maxillary first molars in subjects with the age of 18-49 years are classified as type II,and most of the mesiobuccal(MB)root canals of maxillary first molars in subjects with the age of 50-69 years are classified as type I.The distobuccal(DB)root canals of maxillary first molars in subjects of all age groups are classified as type I.The palatal(P)root canals are mainly classified as type I(one canal in one root).The mean height of pump chambers decreases with aging.A tangent anatomical relationship is identified between the root tip and the maxillary sinus floor.Conclusion The root canal system of maxillary first molars in Qingdao municipality of China is complex and diverse,presenting various anatomical characteristics of the number of roots,number of root canals,morphology of root canals,height of pulp chambers,and the anatomical relationship between the root tip and the maxillary sinus floor.CBCT is an effective tool to illustrate the anatomical characteristics of various types of root canals of maxillary first molars,providing useful data for c
