摘要
目的 评价住院患者使用达比加群酯所存在的药物相关问题(Drug-related problems,DRPs),为临床合理使用达比加群酯提供参考。方法 采用Granada-II分类法,基于药品说明书、新型口服抗凝药物等相关指南制定出达比加群酯的评价标准,对2020年1月至2021年6月嘉兴市第二医院所有住院患者使用达比加群酯的必要性(DRP1、DRP2)、有效性(DRP3、DRP4)及安全性(DRP5、DRP6)进行评价,分析存在的DRPs情况。结果709份住院患者病例中,有289例患者(40.76%)发生了335次DRPs,涉及66次(19.70%)DRP2,48次(14.33%)DPR4,214次(63.88%)DRP5,7次(2.09%)DRP6。按照Granada-II分类法归类,必要性问题66次(19.70%),有效性问题48次(14.33%),安全性问题221次(65.97%)。结论 住院患者使用达比加群酯的DRPs中,较为突出的是安全性问题,为保障住院患者的用药安全,药师应及时做好药学监护,采取相关措施,提高临床合理用药水平。
Objective To evaluated the drug-related problems(DRPs)of dabigatran etexilate in hospitalized patients,and to provide references for the rational use of dabigatran etexilate in clinical practice.Methods The Granada-II classification method was used to develop the evaluation criteria for dabigatran etexilate through the drug instructions and the relevant guidelines for new oral anticoagulant drugs.The necessity(DRP1,DRP2),effectiveness(DRP3,DRP4)and safety(DRP5,DRP6)of dabigatran etexilate used by all patients in the Second Hospital of Jiaxing during hospitalization from January 2020 to June 2021 were evaluated,and the existing DRPs were analyzed.Results Among the 709 inpatient cases,289 patients(40.76%)experienced 335 DRPs,including 66 times of DRP2(19.70%),48 times of DRP4(14.33%),214 times of DRP5(63.88%)and 7 times of DRP6(2.09%).According to the Granada-II classification method,there were 66 necessity questions(19.70%),48 effectiveness questions(14.33%),and 221 safety questions(65.97%).Conclusion Among the inpatients,DRPs of dabigatran etexilate are more prominent in terms of safety.In order to ensure the safety of drug use in the inpatients,pharmacists should do a good job in pharmaceutical monitoring in time and take relevant measures to improve the clinical rational use of drugs.
