摘要
目的结合国际功能、残疾和健康分类(ICF)康复理念对重庆市部分残疾人家庭医生签约机构的服务现状进行问卷调查,并根据其服务执行短板给出合理化建议。方法基于特定情景康复胜任力进行现场问卷调查,获悉59个残疾人家庭医生签约机构科室负责人对ICF六要素的熟悉、应用情况,以及该机构的服务项目开展情况,调查时间为2023年4月。调查结束后,运用Office Excel 2016进行数据录入及处理。结果现场共发放问卷59份,现场回收有效问卷59份,有效回收率为100%。37.3%的残疾人家庭医生签约机构科室负责人完全不了解ICF六要素的具体内容,40.7%的家庭医生签约服务机构转介性服务局限在2项以内,11.9%的家庭医生签约服务机构未执行服务记录书写项目,32.2%的家庭医生签约服务机构目前尚无康复场地,40.7%的家庭医生签约服务机构未配置康复训练器材与辅助器具,至少80.0%的家庭医生签约服务机构未开展各年龄段视力残疾、听力残疾、智力残疾康复服务以及0~6岁肢体残疾的康复服务。结论大多数残疾人签约服务机构科室负责人缺乏ICF理念,多数服务机构存在转介性服务项目单一、服务记录书写不规范、康复专业人员不足,康复场地设备缺乏以及康复服务项目开展数量较少等问题。建议加强家庭医生服务机构科室负责人与其他相关医务人员对ICF理念的学习与服务记录书写的规范化培训;加大对相关专职人员的招募,并促进家庭医生服务机构的资源整合,推动基层康复服务岗位人员的职业化培训。
Objective Combining the rehabilitation concept of the International Classification of Functioning,Disability and Health(ICF),this study conducted a questionnaire survey on the current service status of some family doctor contracting institutions for disabled people in Chongqing and provided rational suggestions based on the shortcomings in service implementation.Methods Based on the specific context of rehabilitation competence,on-site questionnaire surveys were conducted to understand the familiarity and application level of the six elements of ICF by department heads of 59 family doctor contracting institutions for disabled people,as well as the implementation of service projects in these institutions.The survey was conducted in April 2023.After the survey,Office Excel 2016 was used for data entry and processing.Results A total of 59 questionnaires were distributed on-site,and 59 valid questionnaires were collected on-site,with an effective recovery rate of 100%.Overall,37.3%of the department heads of family doctor contracting institutions for disabled people had no knowledge of the specific content of the six elements of ICF;40.7%of these institutions limited their referral services to no more than two items;11.9%of these institutions did not implement service record writing.32.2%of the family doctor contracting service institutions currently do not have rehabilitation facilities,and 40.7%of these institutions do not have rehabilitation training equipment and assistive devices.At least 80.0%of these institutions do not provide rehabilitation services for visual disabilities,hearing disabilities,intellectual disabilities in all age groups,and physical disabilities in children aged 0-6.Conclusion Most department heads of family doctor contracting service institutions for disabled people lack the concept of ICF.Most service institutions face problems such as single referral service projects,non-standard service record writing,insufficient rehabilitation professionals,lack of rehabilitation facilities,and a small
