摘要
机械通气技术是重症监护室(intensive care unit,ICU)患者救治中的常用手段,但长期使用机械通气可能会发生ICU获得性衰弱、呼吸机相关性肺炎、呼吸肌无力、压疮等一系列并发症的概率,延长机械通气时间,增加病死率。因此,早期活动训练对于ICU机械通气患者的康复非常重要,同时也是综合治疗中的重要组成部分。早期活动训练可以改善膈肌功能、通气状态,增强肌肉力量,有助于ICU机械通气患者尽早脱机,缩短机械通气时间和住院时间。此外,早期活动训练还可以减少呼吸机相关性肺炎和卧床时间,改善肺部功能,提高生活质量。本文就ICU机械通气患者早期活动训练的必要性、时机、安全性、有效性、方案及效果方面进行综述,为ICU机械通气患者早期活动训练提供参考。
Mechanical ventilation is a common treatment of patients in intensive care unit(ICU).However,a long-term use of mechanical ventilation may lead to a series of complications,such as ICU-acquired weakness,ventilator-associated pneumonia,respiratory myasthenia,pressure sores,etc.,prolong duration of mechanical ventilation,and increase mortality.Therefore,early activates are very important for the rehabilitation of ICU patients with mechanical ventilation,which are also an important part of comprehensive treatment.Early activities can improve diaphragm function and ventilation state,enhance muscle strength,and shorten ventilation time and length of stay by an early wean off.In addition,early activities can also reduce the incidence of ventilator-associated pneumonia and duration of bed rest,and improve lung function and the quality of life.This article summarized the necessity,timing,safety,effectiveness,scheme and effect of early activities for ICU patients with mechanical ventilation,in order to provide references for early activities for them.
