摘要
目的探讨变应性鼻炎(AR)患者的咽鼓管功能特点及其与鼻部症状的相关性,并观察合并咽鼓管功能障碍(ETD)的AR患者行皮下特异性免疫治疗(SCIT)前后咽鼓管功能的变化。方法对25例健康志愿者(对照组)和50例接受SCIT治疗的AR患者(观察组)进行咽鼓管测压(TMM)、咽鼓管功能障碍问卷-7(ETDQ-7)调查和鼻部症状(鼻塞、鼻痒、喷嚏、流涕)VAS评分。其中24例合并ETD的AR患者在治疗6个月和12个月后再次进行相同测试。结果观察组TMM评分低于对照组,ETDQ-7评分高于对照组,差异有统计学意义(P<0.05)。治疗前AR患者鼻塞症状与TMM呈负相关(r=-0.283,P=0.004),与ETDQ-7评分呈正相关(r=0.395,P=0.005)。合并ETD的AR患者,在SCIT治疗后6个月和12个月TMM评分与治疗前比较明显升高(P<0.05或P<0.01);ETDQ-7评分与治疗前比较明显下降(P<0.05或P<0.01)。结论AR患者常伴有ETD,鼻塞症状与ETD存在相关性。SCIT治疗随时间的增加,改善鼻部症状的同时促进咽鼓管功能的恢复。
Objective To study the characteristics of eustachian tube function in allergic rhinitis(AR)and its correlation with nasal symptoms.To observe the changes of eustachian tube function in AR patients with eustachian tube dysfunction(ETD)before and after the subcutaneous specific immunotherapy(SCIT).Methods 25 healthy volunteers(the control group)and 50 AR patients receiving SCIT(the study group)were subjected to tubomanometry(TMM),7-item Eustachian Tube Dysfunction Questionnaire(ETDQ-7),and VAS scores of nasal symptoms(nasal congestion,itching,sneezing,and runny nose).Among them,24 AR patients with ETD underwent the same test again at 6 and 12 months after SCIT.Results The TMM score of AR patients was lower than that of the control group,and the ETDQ-7 score was higher than that of the control group,the differences were statistically significant(P<0.05).The nasal congestion symptoms of AR patients before SCIT were negatively correlated with TMM(r=-0.283,P=0.004),and positively correlated with ETDQ-7 score(r=0.395,P=0.005).AR patients with combined ETD showed a significant increase in TMM scores at 6 and 12 months after SCIT treatment compared to those before the treatment(P<0.05 or P<0.01).The ETDQ-7 score significantly decreased compared to those before the treatment(P<0.05 or P<0.01).Conclusion AR patients are often accompanied by ETD,and nasal congestion is related to ETD.SCIT can improve nasal symptoms and promote the recovery of eustachian tube function with the increase of time.
