摘要
目的通过观察健脾祛痰法对脾虚痰浊型血脂异常患者的血脂水平、中医证候积分的影响,分析高脂人群采用健脾祛痰法治疗前后肠道菌群的结构变化,检测精炼方颗粒剂中绞股蓝皂苷A、毛蕊异黄酮葡萄糖苷、黄芪甲苷的含量,以明确健脾祛痰法降脂的临床疗效,探究健脾祛痰法发挥临床疗效的有效药理成分。方法在辽宁中医药大学附属医院纳入脾虚痰浊血脂异常受试者共计120例,按照就诊顺序随机分为模型组、治疗组,每组60例。两组均予健康宣教,治疗组予精炼方颗粒剂冲服,疗程为2个月。采用全自动生化分析仪检测所有受试者血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平,用中医证候积分量表评定患者中医证候疗效,16S rRNA高通量测序技术检测受试者粪便标本肠道菌群的结构变化。结果治疗组患者血脂水平、中医证候积分较治疗前明显改善,血脂疗效、中医证候总有效率显著优于模型组。治疗组患者用药后肠道菌群样本间差异明显,各分类水平物种组成比例发生变化。用药后肠道中的肠杆菌目(Enterobacteriales)减少,肠道菌群平衡有所恢复。结论健脾祛痰法能够显著降低血脂异常患者TC、LDL水平,改善中医证候,恢复肠道菌群平衡,并通过绞股蓝皂苷A、毛蕊异黄酮葡萄糖苷、黄芪甲苷等活性成分发挥降脂作用。
Objective To observe the effect of strengthening spleen and expelling phlegm method on blood lipid levels and TCM syndrome scores in patients with dyslipidemia due to spleen deficiency and phlegm turbidity type,and to analyze the structural changes of the intestinal flora of the patients with high-fat population before and after strengthening spleen and expelling phlegm method.To determine the contents of Gynostemma saponin A,Verisoflavone glucoside,and Astragaloside Ⅳ in Jinglianfang Granules(精炼方颗粒),in order to clarify the clinical efficacy of strengthening spleen and expelling phlegm method for lowering lipids,and to explore the effective pharmacological components of strengthening spleen and expelling phlegm method to exert clinical curative effect. Methods A total of 120 subjects with spleen deficiency and phlegm turbidity type dyslipidemia were enrolled in the affiliated hospital of Liaoning university of traditional Chinese medicine. They were randomly divided into a model group and a treatment group according to the order of treatment,with 60 cases in each group. All groups were given health education,and the treatment group was given Jinglianfang Granules for ingestion. The course of treatment was 2 months. Serum cholesterol (TC),triglyceride(TG),low-density lipoprotein(LDL-C),and high-density lipoprotein(HDL-C) levels of all subjects were detected by an automatic analyzer,and the TCM symptom score scale was used to evaluate the efficacy of TCM syndromes in patients,16SrRNA high-throughput sequencing technology was used to detect the structural changes of intestinal flora in fecal samples of subjects. Results The blood lipid levels and TCM syndrome scores of the treatment group were significantly improved compared with those before treatment,and the efficacy of blood lipids and the total effective rate of TCM syndrome scores were significantly better than those of the model group.
