摘要
目的探讨丹青胶囊联合阿维A胶囊治疗寻常型银屑病血瘀证的临床研究。方法选择2022年1—6月本院门诊治疗的62例寻常型银屑病血瘀证患者作为研究对象,按照随机数表法分为对照组和治疗组两组,每组各31例。对照组采用阿维A胶囊治疗,20 mg/次,1次/d;治疗组在对照组治疗的基础上给予口服丹青胶囊,4粒/次,3次/d,两组共计治疗8周。比较两组治疗后临床疗效、PASI和DLQI评分、Th1、Th2、Th17和Treg细胞百分比及其分泌的细胞因子表达水平的差异。结果8周后,与对照组相比,治疗组总有效率明显升高(96.77%vs.77.42%,χ2=5.167,P=0.023)。治疗后,与对照组相比,治疗组PASI评分[(3.23±0.84)分vs.(7.23±1.02)分]、DLQI[(12.04±3.11)分vs.(15.75±2.97)分]、Th1细胞百分比[(3.74±0.26)%vs.(5.26±0.48)%]、Th1/Th2细胞比值[(1.16±0.13)vs.(1.77±0.10)]、Th17细胞百分比[(5.04±0.63)%vs.(8.17±1.02)%]、Th17/Treg细胞比值[(0.63±0.07)vs.(1.14±0.05)]、IFN-g[(8.74±2.21)pg/ml vs.(15.63±2.79)pg/ml]和IL-17[(11.59±1.53)pg/ml vs.(19.74±1.16)pg/ml]表达水平明显降低,差异均具有统计学意义(P<0.05),而Treg细胞百分比[(8.17±1.25)vs.(6.38±0.69)]、IL-4[(133.04±20.71)pg/ml vs.(102.67±17.52)pg/ml]和TGF-α[(40.55±4.36)pg/ml vs.(32.35±3.49)pg/ml]表达水平明显升高,差异均具有统计学意义(P<0.05)。对照组和治疗组不良反应发生率分别为16.13%和22.58%,组间差异无统计学意义(χ2=0.413,P=0.520)。结论丹青胶囊治疗寻常型银屑病血瘀证临床疗效显著,安全性高,可能与改善Th1/Th2和Th17/Treg细胞失衡有关。
Objective To investigate the clinical study of treating psoriasis vulgaris of blood-stasis syndrome with Danqing capsule combined with Acitretin capsule.Methods A total of 62 patients with psoriasis vulgaris of blood-stasis syndrome who were admitted to our hospital from January 2022 to June 2022 were selected as study subjects.According to the random number table method,all the patients were randomly divided into the control group(n=31)and the treatment group(n=31).The control group were treated with Acitretin Capsule,20 mg/time,one time/day,and patients in the treatment group were treated with with Danqing capsule(4 capsules/time,three times/day)based on the control group.All patients were treated for 8 weeks.The differences of clinical efficacy,scores of PASI and DLQI,Th1 cell percentage,Th2 cell percentage,Th17 cell percentage,Treg cell percentage and the related cytokines levels after treatment were compared between the two groups.Results After 8 weeks of treatment,the total effective rate of the treatment group was significantly higher than the control group(96.77%vs.77.42%,χ2=5.167,P=0.023).After treatment,when compared with the control group,the PASI score[(3.23±0.84)vs.(7.23±1.02)],DLQI[(12.04±3.11)vs.(15.75±2.97)],Th1 cell percentage[(3.74±0.26)vs.(5.26±0.48)],Th1/Th2 cell ratio[(1.16±0.13)vs.(1.77±0.10)],Th17 cell percentage[(5.04±0.63)vs.(8.17±1.02)],Th17/Treg cell ratio[(0.63±0.07)vs.(1.14±0.05)],serum IFN-g level[(8.74±2.21)vs.(15.63±2.79)]and serum IL-17 level[(11.59±1.53)vs.(19.74±1.16)]were significantly decreased,while the Treg cell percentage[(8.17±1.25)vs.(6.38±0.69)],serum IL-4 level[(133.04±20.71)vs.(102.67±17.52)]and serum TGF-αlevel[(40.55±4.36)vs.(32.35±3.49)]were significantly increased.All the differences were statistically significant(P<0.05).The incidence of adverse reactions rate both in the control group and the treatment group were 16.13%and 22.58%,respectively.There was no significant difference between the two groups(χ2=0.413,P=0.520).Conclusions Danqing
