摘要
目的分析MRI对肺癌淋巴结转移的诊断价值。方法60例肺癌患者均行MRI检查,以手术病理结果为金标准,分析MRI对肺癌淋巴结转移的诊断效能。根据手术病理结果将60例患者分为转移组和未转移组,比较两组患者的MRI相关参数,采用受试者工作特征(ROC)曲线分析MRI各参数对肺癌淋巴结转移的诊断效能。结果以手术病理结果作为金标准,MRI诊断肺癌淋巴结转移的灵敏度为88.57%(31/35),特异度为96.00%(24/25),准确度为91.67%(55/60),MRI诊断肺癌淋巴结转移与手术病理结果高度一致(Kappa=0.690)。转移组患者容积转移常数(K^(trans))、血管外细胞外间隙容积分数(Ve)、病灶/脊髓信号比值(LSR)、标准摄取值(SUV)均明显高于未转移组,表观弥散系数(ADC)明显低于未转移组,差异均有统计学意义(P﹤0.01)。ROC曲线分析发现,Ktrans、Ve、ADC、LSR和SUV诊断肺癌淋巴结转移的灵敏度和特异度均较高。结论MRI对肺癌淋巴结转移的诊断效能高。
Objective To analyze the diagnostic value of MRI in lymph node metastasis of lung cancer.Method Sixty patients with lung cancer were examined by MRI.The diagnostic efficacy of MRI for lymph node metastasis of lung cancer was analyzed by surgical pathology as the gold standard.Sixty patients were divided into metastasis group and non-metastasis group according to the results of surgical pathology,the MRI related parameters of the two groups were compared.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of MRI parameters for lymph node metastasis of lung cancer.Result By surgical pathology as the gold standard,the sensitivity,specificity and accuracy of MRI in the diagnosis of lymph node metastasis of lung cancer were 88.57%(31/35),96.00%(24/25),91.67%(55/60),respectively.The result of MRI in the diagnosis of lymph node metastasis of lung cancer was highly consistent with the result of surgical pathology(Kappa=0.690).The volume transfer constant(Ktrans),extravascular extracellular volume fraction(Ve),lesion-to-spinal cord ratio(LSR),and standardized uptake value(SUV)in the metastasis group were significantly higher than those in the non-metastasis group,and the apparent diffusion coefficient(ADC)was significantly lower than that in the non-metastasis group,the differences were statistically significant(P<0.01).ROC curve showed that Ktrans,Ve,ADC,LSR and SUV had high sensitivity and specificity in the diagnosis of lymph node metastasis of lung cancer.Conclusion MRI has a high diagnostic efficiency for lymph node metastasis of lung cancer.
