新形势背景下山东省CT设备合理配置规划研究

摘要 目的:探究分析新形势背景下山东省各地市大型影像设备配置使用现状,针对64排及以上CT被调出管理品目,预测分析其配置规划。方法:采用德尔菲专家咨询法确定调研问卷,通过山东省大型医用设备配置管理系统进行问卷定向发放,在山东省内进行大型医用设备摸底调研,基于卫生经济学理论对山东省各地市医疗机构现有CT设备配置及使用的经济性和检查的有效性进行分析,采用大型医用设备配置预测模型对各地市的配置规划进行预测。结果:山东省各地市医疗机构现有CT设备717台,其中三级医院427台,二级医院264台,其他医疗机构26台。山东省各地市医疗机构CT设备存在分布地域不均衡现象,人均台数最多的为济南(0.12台/万人),最少为菏泽(0.05台/万人);设备使用中患者平均等待时间最长为淄博(1.26 d),最短为东营(0.13 d);CT检查结果阳性病例数年均值为97.08万人次,年检查结果阳性病例人数最多的为济南(377.02万人次),阳性检查率为84.90%;CT日平均开机时间为17.22 h,日平均开机时间最长的为聊城(19.66 h),最短为青岛(14.93h)。基于预测模型需配置数量前3位城市分别为济南(169台)、临沂(151台)和青岛(136台)。结论:基于卫生经济学理论框架结合大型医用设备配置预测模型对新形势背景下山东省各地市的大型医用设备配置数量进行预测,能够为各医疗机构科学配置大型医疗设备和政府机构监管方案制定提供参考依据。 Objective:Based on the CT of 64-slice and above being called out of the management items,to explore and analyze the allocation and use of large-scale imaging equipment in various cities in Shandong province under the background of the new situation,and to carry out allocation planning forecast analysis.Methods:The Delphi expert consultation method was used to determine the survey questionnaire,the questionnaires were distributed through the large-scale medical equipment allocation and management system of Shandong province,and a thorough survey of large-scale medical equipment was conducted in Shandong Province.Based on the theory of health economics,the economy of the existing CT equipment allocation and effectiveness of examinations in medical institutions in various cities in Shandong Province were analyzed,and the allocation forecast model of large-scale medical equipment was used to forecast the allocation planning of various cities.Results:A total of 717 units of CT equipment in medical institutions in various cities in Shandong Province,including 427 units in tertiary hospitals,264 units in secondary hospitals,and 26 units in other medical institutions.There was a geographical imbalance in the distribution of CT in Shandong Province,as the most per capita units were 0.12 units/10,000 people in Jinan,and the least was 0.05 units/10,000 people in Heze.The longest average waiting time of patients with equipment was(1.26 days)in Zibo and the shortest is(0.13 day)in Dongying.The annual average of positive CT examination cases was 970,800 person-times,and Jinan had the highest number of positive cases(3,770,200 persontimes)with a positive examination rate of 84.90%.The average daily startup time of CT was 17.22 hours,the longest daily average startup was 19.66 hours in Liaocheng,and the shortest was 14.93 hours in Qingdao.Based on the forecast model,the top three cities in terms of allocations required were 169 in Jinan,151 in Linyi and 136 in Qingdao.Conclusion:Based on the theoretical framework of health econo
