摘要
目的 调查住院患者人纤维蛋白原使用情况,分析我国人纤维蛋白原使用现状,为临床合理用药提供参考。方法 随机抽取中国9地区120家医院2015年1月1日—2020年12月31日住院患者使用人纤维蛋白原的处方,对处方量、药品使用数量、药品费用、科室分布及诊断分布等进行分析。结果 9地区间纳入医院类型差异无统计学意义(P>0.05);共抽取处方29 245张,人纤维蛋白原产品来源主要为国产;处方量随时间变化总体呈增长趋势,其中增长趋势较为显著是成都、广州、上海和天津4地区;平均每年每家医院处方量、药品使用数量及药品费用位列前5的地区依次是杭州、广州、北京、成都和上海,最低的地区是哈尔滨;处方量、药品使用数量和药品费用方面位列前3的科室依次是重症监护室、心胸外科和血液外科;位列前3的诊断依次是心血管系统疾病、消化系统疾病和肿瘤。结论 我国人纤维蛋白原使用存在地区差异,南方多于北方;各地区处方量呈逐年增加趋势;疾病诊断主要集中在心血管系统疾病、消化系统疾病和肿瘤。
Objective To investigate the use of human fibrinogen in inpatients,to analyze the current situation of the use of human fibrinogen in China,and to guide rational drug use in clinics.Methods The prescriptions of human fibrinogen used by inpatients in 120 hospitals in 9 regions of China from January 1,2015 to December 31,2020 were randomly selected.The number of prescriptions/drugs used,the cost,the distribution of departments,and the distribution of diagnoses were analyzed.Results There was no statistically significant difference in the type distribution of included hospitals among nine regions(P>0.05).Twenty-nine thousand two hundred forty-five prescriptions were extracted,and human fibrinogen product sources were mainly domestic.The number of drugs in the nine regions showed an increasing trend over time,which was more significant in Chengdu,Guangzhou,Shanghai,and Tianjin.Comparing the average annual prescription quantity,drug quantity,and prescription amount of each hospital in 9 regions,the top five regions were Hangzhou,Guangzhou,Beijing,Chengdu,and Shanghai;and Harbin was the lowest.ICU,cardiothoracic,and hematology surgery ranked in the top 3 in prescription volume,drug quantity,and amount.The top three diagnoses were cardiovascular disease,digestive system disease,and cancer.Conclusion There are regional differences in the use of human fibrinogen,more in the south than in the north.The amount of human fibrinogen used in our country increased year by year.The diseases covered were mainly cardiovascular diseases,digestive diseases,and tumors.
