摘要
目的 分析拉贝洛尔联合尼卡地平治疗妊娠期高血压的临床效果及对凝血功能的影响。方法 将2020年1月至2022年12月收治的86例妊娠期高血压患者根据治疗方案分为常规组和观察组,每组43例。常规组给予尼卡地平治疗,观察组在常规组治疗基础上增加拉贝洛尔治疗。比较两组的治疗效果。结果 观察组的治疗总有效率显著高于常规组(P<0.05)。治疗后,观察组的24 h尿蛋白定量(24 h UPQ)、外周血管阻力、中心静脉压小于常规组,心脏指数大于常规组(P<0.05)。治疗后,观察组的D-二聚体(D-D)、纤维蛋白原(FIB)水平显著低于常规组,凝血酶时间(TT)、凝血酶原时间(PT)及活化部分凝血活酶时间(APTT)显著长于常规组(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05);观察组的不良妊娠结局发生率显著低于常规组(P<0.05)。结论 拉贝洛尔联合尼卡地平治疗妊娠期高血压可有效消除疾病症状,调节血流动力学与凝血功能,减少不良妊娠结局,且不会增加不良反应,值得应用。
Objective To analyze the clinical effect of labetalol combined with nicardipine in the treatment of gestational hypertension and its influence on coagulation function.Methods A total of 86 patients with gestational hypertension admitted from January 2020 to December 2022 were divided into routine group and observation group according to the treatment plan,with 43 cases in each group.The routine group was treated with nicardipine,and the observation group was treated with labetalol on the basis of the routine group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the routine group(P<0.05).After treatment,the 24 h urinary protein quantification(24 h UPQ),peripheral vascular resistance and central venous pressure in the observation group were less than those in the routine group,and the cardiac index was greater than that in the routine group(P<0.05).After treatment,the levels of D-Dimer(D-D)and fibrinogen(FIB)in the observation group were significantly lower than those in the routine group,and the thrombin time(TT),prothrombin time(PT)and activated partial thromboplastin time(APTT)were significantly longer than those in the routine group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05);the incidence of adverse pregnancy outcomes in the observation group was significantly lower than that in the routine group(P<0.05).Conclusion Labetalol combined with nicardipine in the treatment of gestational hypertension can effectively eliminate the symptoms of the disease,regulate hemodynamics and coagulation function,reduce adverse pregnancy outcomes,and will not increase adverse reactions,which is worthy of application.
