基于肠道微生物学及代谢组学研究2型糖尿病患者肠道菌群结构及血清代谢产物的特征 被引量:1

Characteristics of intestinal microbiota structure and serum metabolites in patients with type 2 diabetes based on gut microbiology and metabolomics:a cohort study

摘要 目的探究2型糖尿病(type 2 diabetes,T2DM)患者肠道菌群特征及血清代谢指标与健康人群之间的差异,分析血清代谢组学特征与肠道菌群特征之间的相关性。方法纳入2021年6月至2022年12月新疆维吾尔自治区妇幼保健院就诊的100例初发T2DM患者作为DM组;同期招募100例健康志愿者作为对照组。收集两组对象粪便和血清标本,采用高通量测序技术对粪便标本进行16S rRNA测序,分析T2DM患者肠道菌群整体结构及多样性特征,比较两组间的显著差异物种,运用PICRUSt工具对肠道差异菌群进行基因功能通路预测。采用GC-MS技术对两组对象血清标本进行代谢谱测定,分析两组对象血清代谢物之间的差异;利用Mectaboanalyst软件分析血清差异代谢物涉及的代谢通路;将T2DM患者血清中的差异代谢物与高通量测序得到的肠道菌群代谢通路数据进行相关性分析。结果肠道菌群α多样性方面,DM组Chao指数、Ace指数、Shannon指数低于对照组,Simpson指数高于对照组,差异有统计学意义(P<0.05);两组肠道菌群β多样性差异有统计学意义(P<0.05)。DM组患者肠道菌群中以肠球菌属(Enterococcus)、梭菌属(Clostridioides)、粪杆菌属(Faecalibacterium)、放线菌属(Actinomyces)、埃希菌属(Escherichia)、伊格尔兹菌属(Eggerthella)、安德克菌属(Adlercreutzia)占主要优势;对照组肠道菌群中以拟杆菌门(Bacteroidetes)、乳杆菌属(Lactobacillus)、双歧杆菌属(Bifidobacterium)、副杆菌属(Parabacteroides)、普雷沃菌(Prevotella)、甲烷短杆菌属(Methanobrevibacter)、二十碳孢梭菌属(Erysipelatoclostridium)占主要优势。代谢组学方面,DM组乳酸、亚油酸、棕榈酸、油酸水平高于对照组,L-丙氨酸、L-缬氨酸、络氨酸水平低于对照组,差异有统计学意义(P<0.05);PICRUSt工具对肠道差异菌群进行基因功能通路预测显示,肠道差异菌属可获得3个代谢通路信息,分别是其他次级代谢产物的合� Objective To explore the characteristics of intestinal microbiota and the differences in serum meta-bolic indicators between patients with type 2 diabetes(T2DM)and healthy individuals,and analyze the correlation be-tween serum metabolomic features and gut microbiota characteristics.Methods A total of 100 newly diagnosed T2DM patients treated at the Xinjiang Uygur Autonomous Region Maternal and Child Health Hospital from June 2021 to December 2022 were enrolled as the DM group.Concurrently,100 healthy volunteers were recruited as the control group.Stool and serum specimens were collected from both groups.High-throughput sequencing technology was used for 16S rRNA se-quencing of stool samples to analyze the overall structure and diversity of the gut microbiota in T2DM patients.Significant differences in species between the two groups were compared,and the PICRUSt tool was used to predict the gene functional pathways of dfferent gut microbial communities.GC-MS technology was employed for metabolic spectrum determination of serum samples from both groups.The differences in serum metabolites between the two groups were analyzed.The Mectaboanalyst software was used to analyze the metabolic pathways involved in different serum metabolites.Correlation analysis was conducted between different metabolites in the serum of T2DM patients and the metabolic pathway data of gut microbiota obtained through high-throughput sequencing.Results In terms ofα-diversity of the gut microbiota,the Chao index,Ace index,and Shannon index of the DM group were lower than those of the control group,while the Simpson index was higher(all P<0.05).Significant statistical differences were observed in theβ-diversity of the gut microbiota between the two groups(P<0.05).The dominant genera in the gut microbiota of the DM group included Enterococcus,Clostridioides,Faecalibacterium,Actinomyces,Escherichia,Eggerthella,and Adlercreutzia.In contrast,the control group had predominance of Bacteroidetes,Lactobacllus,Bifidobacterium,Parabacteroides,Prevotel
