肺炎克雷伯菌致下呼吸道感染患者常用抗菌药物的耐药性分析

摘要 目的分析肺炎克雷伯菌(Klebsiella pneumoniae,KPN)致下呼吸道感染(lower respiratory tract infection,LRI)患者对常用抗菌药物的耐药性。方法选取2022年10月至2023年9月我院KPN致LRI患者514例,采集痰标本,统计KPN、超广谱β-内酰胺酶(EBLS)+菌株、EBLS菌株、耐碳青霉烯类肺炎克雷伯菌(CRKP)检出情况,行药敏试验,分析KPN、EBLS+菌株、EBLS菌株及CRKP菌株对常用抗菌药物的耐药性。结果分离出514株KPN,其中EBLS+菌株94株(18.3%)、EBLS菌株310株(60.3%)、CRKP菌株110株(21.4%);KPN对替加环素最为敏感,达99.8%;EBLS+菌株对大部分常用抗菌药物的耐药率较EBLS菌株高;除替加环素外,CRKP菌株对其他常用抗菌药物的耐药率均较高,>80.0%。结论KPN致LRI患者中分离出EBLS+菌株、EBLS菌株及CRKP菌株,不同菌株对不同抗菌药物耐药率不同,需进行细菌培养与鉴定,了解其耐药率,帮助临床医师合理选择抗菌药物进行治疗。 Objective To analyze the resistance to common antibiotics in patients with lower respiratory infection(LRI)caused by Klebsiella pneumoniae(KPN).Methods A total of 514 patients with LRI caused by KPN in our hospital from October 2022 to September 2023 were selected,sputum samples were collected,and the detection status of KPN,extended spectrumβ-lactamases(EBLS)+strains,EBLS-strains and carbapenem resistant Klebsiella pneumoniae(CRKP)were analyzed.Drug sensitivity tests were performed to analyze the resistance of KPN,EBLS+strains,EBLS-strains and CRKP strains to commonly used antimicrobial drugs.Results A total of 514 KPN strains were isolated,including 94 EBLS+strains(18.3%),310 EBLS-strains(60.3%)and 110 CRKP strains(21.4%).KPN was the most sensitive to tigecycline(99.8%).The resistance rate of EBLS+strain to most commonly used antibiotics was higher than that of EBLS-strain.Except for tigecycline,CRKP strains had a high resistance rate to all commonly used antimicrobials,>80.0%.Conclusion EBLS+strains,EBLS-strains and CRKP strains are isolated from patients with LRI caused by KPN,and different strains have different rates of resistance to different antimicrobial drugs,which need to be cultured and identified in order to understand the rate of resistance,and to help clinicians rationally select antimicrobial drugs for treatment.
