摘要
目的探讨耳后注射甲泼尼龙琥珀酸钠治疗突发性耳聋的疗效及对患者血清同型半胱氨酸(Hcy)水平、血小板计数与不良反应的影响。方法选取2019年1月至2021年12月南昌市第一医院收治的60例突发性耳聋患者作为研究对象,采用交替分组法随机分为研究组与对照组,每组30例。两组均实施常规治疗,在此基础上,对照组使用地塞米松磷酸钠注射液治疗,研究组使用注射用甲泼尼龙琥珀酸钠治疗。比较两组Hcy水平、血小板计数、纯音听阈值、血流动力学情况、凝血功能情况及不良反应发生情况。结果治疗后,研究组Hcy水平、血小板计数及纯音听阈值均低于对照组,差异有统计学意义(P<0.05);研究组血小板聚集率、血细胞比容及血浆黏度均低于对照组,差异有统计学意义(P<0.05);研究组活化部分凝血酶时间(APTT)、凝血酶原时间(PT)均长于对照组,血小板因子(PF)水平低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论耳后注射甲泼尼龙琥珀酸钠治疗突发性耳聋的疗效显著,可有效降低患者血清Hcy水平及血小板计数,且安全性较高。
Objective To investigate the efficacy of postauricular injection of methylprednisolone sodium succinate in the treatment of sudden deafness and its effect on serum homocysteine(Hcy)level,platelet count and adverse reactions.Methods 60 patients with sudden deafness admitted to Nanchang First Hospital from January 2019 to December 2021 were selected as the research subjects,and they were randomly divided into the study group and the control group by alternate grouping method,with 30 cases in each group.Both groups received conventional treatment,on this basis,the control group was treated with dexamethasone sodium phosphate injection,and the study group was treated with methylprednisolone sodi-um succinate for injection.The Hcy level,platelet count,pure tone hearing threshold,hemodynamics,coagulation function and adverse reactions were compared between the two groups.Results After treatment,the Hcy level,platelet count and pure tone hearing threshold in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The platelet aggregation rate,hematocrit and plas-ma viscosity in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Activated par-tial thrombin time(APTT)and prothrombin time(PT)in the study group were longer than those in the control group,and platelet factor(PF)levels was lower than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Methylprednilone sodium succinate injected retroauricular is more effective and can effec-tively reduce serum Hcy level and platelet count,which is worthy of clinical promotion and application.
