摘要
目的分析凝血项目和糖脂代谢项目检测在妊娠期糖尿病患者中的辅助诊断作用价值。方法选取50例妊娠期糖尿病患者作为观察组;另选取同期50例健康孕妇作为对照组。检测并比较两组凝血指标和糖脂代谢指标。结果与对照组比较,观察组的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、小而密低密度脂蛋白(sdLDL)、糖化血红蛋白(HbA1c)水平较高,高密度脂蛋白胆固醇(HDL-C)水平较低(P<0.05)。与对照组比较,观察组的纤维蛋白原(FIB)、D-二聚体(D-D)指标较高,凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)指标较低(P<0.05)。联合检测对疾病的阳性检出率高于凝血功能指标单独检测(P<0.05)。联合检测的诊断准确率均高于凝血功能、糖脂代谢指标单独检测,敏感度、特异度均高于凝血功能指标单独检测(P<0.05)。结论妊娠期糖尿病患者的凝血指标和糖脂代谢指标与健康孕妇比较存在显著差异,检测妊娠期妇女的凝血指标和糖脂代谢指标可有效辅助诊断妊娠期糖尿病,为妊娠期糖尿病治疗提供参考依据,值得临床推广与应用。
Objective To analyze the diagnostic value of coagulation items and glycolipid metabolism items in gestational diabetes mellitus.Methods 50 patients with gestational diabetes mellitus were selected as the observation group.Another 50 healthy pregnant women were selected as control group.Blood coagulation index and glucose lipid metabolism index were detected and compared between the two groups.Results Compared with the control group,the levels of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),small and dense low-density lipoprotein(sdLDL)and glycosylated hemoglobin(HbA1c)in the observation group were higher,and the levels of high density lipoprotein cholesterol(HDL-C)were lower(P<0.05).Compared with the control group,the indexes of fibrinogen(FIB)and D-dimer(D-D)in observation group were higher,and the indexes of prothrombin time(PT),activated partial thrombin time(APTT)and thrombin time(TT)in observation group were lower(P<0.05).The positive detection rate of the combined test was higher than that of the coagulation function index alone(P<0.05).The diagnostic accuracy of combined detection was higher than that of coagulation function and glycolipid metabolism index alone,and the sensitivity and specificity were higher than that of coagulation function index alone(P<0.05).Conclusion There are significant differences between the coagulation index and glucose and lipid metabolism index of gestational diabetes mellitus patients and healthy pregnant women.Detection of coagulation index and glucose and lipid metabolism index of gestational diabetes mellitus can effectively assist the diagnosis of gestational diabetes mellitus and provide reference for the treatment of gestational diabetes mellitus,which is worthy of clinical promotion and application.
