摘要
目的探究腹腔镜子宫肌瘤剔除术患者实施小剂量艾司氯胺酮与瑞芬太尼联合丙泊酚麻醉诱导的作用。方法选取88例腹腔镜子宫肌瘤剔除术患者,根据随机数字表法分为试验组与对照组,各44例。对照组患者予以瑞芬太尼辅助丙泊酚麻醉,试验组患者在麻醉诱导基础上予以小剂量艾司氯胺酮麻醉。比较两组临床麻醉效果。结果两组患者手术前后收缩压、舒张压水平比较,差异无统计学意义(P>0.05)。两组患者手术前后心率、血氧饱和度和平均动脉压比较,差异无统计学意义(P>0.05)。试验组不同时间段视觉模拟评分法(VAS)评分低于对照组(P<0.05)。两组麻醉用药不良反应发生率比较,差异无统计学意义(P>0.05)。结论在腹腔镜子宫肌瘤剔除术患者麻醉诱导中,联合运用小剂量艾司氯胺酮、瑞芬太尼和丙泊酚,对血压及血流动力学影响轻微,且镇痛效果突出,副作用少。
Objective To investigate the effect of low dose esketamine and remifentanil combined with propofol in patients undergoing laparoscopic myomectomy.Methods 88 patients with laparoscopic myomectomy were selected and divided into experimental group and control group according to random number table method,with 44 cases in each group.The control group was anesthetized with remifentanil and propofol,and the experimental group was anesthetized with small dose of esketamine on the basis of anesthesia induction.The clinical anesthetic effect of the two groups was compared.Results There was no significant difference in the levels of systolic and diastolic blood pressure between the two groups before and after operation(P>0.05).There was no significant difference in heart rate,oxygen saturation and mean arterial pressure between the two groups before and after operation(P>0.05).Visual analogue scale(VAS)scores in experimental groups were lower than those in control group(P<0.05).There was no significant difference in the adverse performance of narcotic drugs between the two groups(P>0.05).Conclusion In the anesthesia induction of patients undergoing laparoscopic myomectomy,the combination of low-dose esketamine,remifentanil and propofol has slight effects on blood pressure and hemodynamics,and the analgesic effect is prominent with few side effects.
