摘要
目的分析在妇科腹腔镜子宫肌瘤剔除术中采用瑞芬太尼复合丙泊酚麻醉的应用价值。方法选取45例妇科腹腔镜子宫肌瘤剔除术患者,通过随机数字表法分为对照组(22例)与观察组(23例)。对照组采用瑞芬太尼复合七氟醚静吸麻醉,观察组采用瑞芬太尼复合丙泊酚麻醉。比较两组患者的麻醉效果。结果与对照组对比,观察组麻醉生效时间、苏醒时间和管道拔除时间均更短,差异有统计学意义(P<0.05)。麻醉诱导前(T_(1))时两组心率(HR)和平均动脉压(MAP)水平对比差异无统计学意义(P>0.05),插管后即可(T_(2))、手术结束时(T_(3))时与对照组对比,观察组HR、MAP水平较低,差异有统计学意义(P<0.05)。观察组不良反应发生率为4.35%,明显低于对照组的27.27%,差异有统计学意义(P<0.05)。结论对妇科腹腔镜子宫肌瘤剔除术患者采用瑞芬太尼复合丙泊酚的麻醉方案能够降低术中血流动力学波动,提升麻醉质量,加快术后苏醒以及减少不良反应发生。
Objective To analyze the application value of remifentanil combined with propofol in gynecological laparoscopic myomectomy.Methods 45 gynecological patients undergoing laparoscopic myomectomy were divided into control group(22 cases)and observation group(23 cases)by random number table method.The control group was given remifentanil combined with sevoflurane inhalation anesthesia,and the observation group was given remifentanil combined with propofol anesthesia.The anesthetic effect of the two groups was compared.Results Compared with the control group,the effective time of anesthesia,recovery time and tube removal time in the observation group were shorter,and the difference was statistically significant(P<0.05).There were no statistically significant differences in heart rate(HR)and mean arterial pressure(MAP)levels between the two groups before anesthesia induction(T_(1))(P>0.05).After intubation(T_(2))and at the end of surgery(T_(3)),the levels of HR and MAP in the observation group were lower,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 4.35%,significantly lower than that in the control group(27.27%),and the difference was statistically significant(P<0.05).Conclusion Remifentanil combined with propofol can reduce hemodynamic fluctuations,improve the quality of anesthesia,accelerate postoperative recovery and reduce the occurrence of adverse reactions in patients undergoing gynecological laparoscopic myomectomy.
