摘要
目的 分析在重症颅脑外伤的治疗中应用双侧相继去骨瓣减压术的临床价值。方法 研究采用随机双盲法将2020年4月—2021年3月高唐县人民医院收治的86例重症颅脑外伤患者分为观察组和对照组各43例。对照组实施双侧同时去骨瓣减压术治疗,同时进行双侧开颅血肿清除和减压;观察组实施双侧相继去骨瓣减压术治疗。比较两组患者治疗效果、GOS评分及治疗安全性。结果 观察组治疗有效率为79.07%,高于对照组的39.53%,差异有统计学意义(P<0.05)。观察组再出血、硬膜下积液并发症发生率均低于对照组,差异均有统计学意义(P<0.05)。两组多器官功能衰竭、脑功能衰竭发生率比较差异无统计学意义(P>0.05)。结论 在重症颅脑外伤的治疗中实施双侧相继去骨瓣减压术能取得较好效果,与双侧同时去骨瓣减压术治疗相比,在提高治疗效果、降低并发症发生率效果明显,有较好的安全性,具有临床应用及推广价值。
Objective To analyze the clinical value of bilateral successive decompressive craniectomy in the treatment of severe craniocerebral trauma.Methods A total of 86 patients with severe craniocerebral trauma treated in Gaotang County People's Hospital from April 2020 to March 2021 were divided into observation group and reference group by random double-blind method,with 43 cases in each group.The reference group was treated with bilateral simultaneous decompressive craniectomy,and bilateral craniotomy hematoma removal and decompression were performed at the same time;the observation group was treated with bilateral successive decompressive craniectomy.The treatment effect,GOS score and treatment safety of the two groups were compared.Results The treatment effective rate in the observation group was 79.07%,which was higher than 39.53% of the reference group,and the difference was statistically significant(P <0.05);the complication rates of rebleeding and subdural effusion in the observation group were lower than those in the reference group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of multiple organ failure and brain failure between the two groups(P>0.05).Conclusion In the treatment of severe craniocerebral trauma,bilateral successive decompressive craniectomy could achieve good results.Compared with bilateral simultaneous decompressive craniectomy,it had significant effect in improving the treatment effect and reducing the incidence of complications,and had good safety.It had significant clinical application and popularization value.
