摘要
报告4例小汗腺汗囊瘤。例1.患者女,48岁。眼睑周围多发丘疹3年。例2.患者女,65岁。面部多发丘疹1年。例3.患者女,63岁。面部多发丘疹2年。例4.患者男,63岁。眼睑多发丘疹4年。皮肤科检查:4例患者均表现为面部多发肤色或淡蓝色的囊性透明丘疹。皮肤镜检查:4例均表现为边界清晰的椭圆形囊性结构,中心为均匀蓝紫色区域,周围包绕苍白色淡晕,其中例3患者可见囊性结构中心为均匀白色,例4患者可见少量分支血管。4例患者皮损组织病理表现相似,均为表皮大致正常,真皮中部可见扩大的囊性导管和腔;囊壁由2层细胞构成,内层为立方形,外层为柱状,细胞核与囊壁平行,部分囊壁呈褶皱或乳头状增生,囊内有淡红色无定形物质,周围可见部分汗腺导管。4例患者均诊断为小汗腺汗囊瘤。
Four cases of eccrine hidrocystoma are reported.Case 1.A 48-years-old female presented with multiple papules on the eyelids for 3 year.Case 2.A 65-year-old female presented with multiple papules on the face for 1 year.Case 3.A 63-year-old female presented with multiple papules on the face for 2 years.Case 4.A 63-year-old male presented with multiple papules on eyelids for 4 years.Physical examination showed that the four patients presented with multiple skin-colored or light blue cystic hyaline papules on the faces.Dermoscopic examination of the four cases showed a well-defined oval cystic structure with a homogeneous bluish-purplish area in the centre,surrounded by a ale halo around,with a uniform white area in the centre of the cystic structure in Case 3,and a few ranch vessels in Case 4.The histopathological manifestations of the lesions of all patients were similar,presenting with the epidermis roughly normal,and enlarged cystic ducts and lumens in the middle part of the dermis.The wall of the sac was composed of two layers of cells;the inner layer was cuboidal and the outer layer was columnar;the nuclei of the cells were parallel to the wall of the sac,and part of the wall of the sac was wrinkled or papillary hyperplasia,with a reddish amorphous substance inside the sac;some of the sweat ducts were visible in the surrounding area.The four patients were diagnosed as eccrine hidrocystoma.
