摘要
目的观察15%壬二酸治疗轻中度寻常痤疮的功效性及安全性。方法自2021年10月至2022年7月,北京大学第一医院皮肤性病科采用自身对照观察治疗效果,纳入33例轻中度寻常痤疮患者,全面部涂抹15%壬二酸,早晚各1次,持续8周。分别在治疗前,第2周,第4周,第8周进行随访,记录面部痤疮炎症性和非炎症性皮损计数、IGA评分、VISIA成像、皮肤生理指标(角质层含水量、经皮水分丢失、pH值、皮脂分泌率)、患者对痤疮改善情况自评并收集不良反应。结果30例获随访8周。与治疗前相比,治疗后第2周面部非炎性痤疮皮损计数减少17.1%(P<0.05),第8周时面部非炎性皮损和炎性皮损分别减少了46.4%和34.3%(P<0.05)。IGA评分随着使用时间延长呈下降趋势(P<0.05),第8周时96.4%的患者自评痤疮得到不同程度的改善。皮肤生理指标检测显示,第2、4、8周皮肤经皮水分丢失和pH值较基线下降(P<0.05),角质层含水量和皮脂分泌率与基线相比,差异无统计学意义(P>0.05)。使用初期部分患者有轻度瘙痒、灼热、刺痛的不适症状;随使用时间延长,以上不适症状均减轻或消失,无患者因不良反应退出研究。结论15%壬二酸可迅速有效地减少非炎性痤疮皮损数量,对于炎症性皮损也有一定效果;其刺激性较小,可用于轻中度寻常痤疮的辅助治疗。
Objective To examine the efficacy and safety of 15%azelaic acid gel inmild tomoderate acne vulgaris.Methods Thirty-three patients with mild to moderate acne vulgaris were enrolled in a single-center,self-controlled study.15%azelaic acid was applied to the entire face twice a day for 8 weeks.All patients were followed up at week 0,2,4 and 8,and the count of inflammatory and non-inflammatory facial acne lesions and the skin biophysical parameters(stratum corneum water content,transepidermal water loss(TEWL),pH value and sebum secretion rate)were measured.The IGA score assessed by doctors,patients'self-assessment and adverse effects were also collected.Results Of the 33 patients,three were lost to follow-up and 30 patients completed the study.Compared with baseline,non-inflammatory lesion count was reduced by 17.1%at week 2(P<0.05).At week 8,non-inflammatory and inflammatory lesion counts were reduced by 46.4%and 34.3%,respectively(P<0.05).IGA scores continuously decreased with the usage of the testing product(P<0.05),and 96.4%patients achieved varying degrees of improvement at the end of follow-up.The TEWL and pH values were lower than baseline at weeks 2,4,and 8(P<0.05),while the stratum corneum water content and sebum secretion rate showed no significant changes compared with baseline(P>0.05).Some patients experienced mild itching,burning and stinging sensation at the early stage of usage,which alleviated or resolved after prolonged usage,and no patient withdrew from the study due to adverse reactions.Conclusion 15%azelaic acid can rapidly and effectively reduce acne lesions,especially for non-inflammatory lesions with less irritation,and can be used as an adjunctive treatment for mild to moderate acne vulgaris.
