摘要
目的探讨左甲状腺素不同初始治疗剂量对妊娠期亚临床甲状腺功能减退(SCH)患者临床疗效及安全性的影响。方法选取2020年1—12月于九江市第三人民医院就诊的60例SCH患者作为研究对象,按照随机数字表法为高剂量组与低剂量组,每组30例。所有患者均给予左旋甲状腺素钠片,高剂量组初始剂量为每天75μg,低剂量组初始剂量为每天25μg。比较两组不同治疗时间血清促甲状腺激素(TSH)水平、并发症及用药不良反应发生情况。结果治疗前,两组TSH水平比较差异无统计学意义;治疗后2、4周,两组TSH水平均低于前一时间点,且高剂量组低于低剂量组,差异有统计学意义(P<0.05)。高剂量组并发症发生率低于低剂量组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论初始治疗时,高剂量左甲状腺素治疗妊娠期SCH疗效显著,可减少妊娠期并发症,改善妊娠结局。
Objective To investigate the efficacy and safety of different initial treatment dosages of levothyroxine in subclinical hypothyroidism(SCH)during pregnancy.Methods 60 paitents with SCH who were treated in Jiujiang Third People's Hospital from January to December 2020 were selected as the research subjects,they were divided into the high-dose group and the low-dose group according to the random number table method,with 30 cases in each group.All patients were given levothyroxine sodium tablets,the initial dose of high-dose group was 75μg per day,and the initial dose of low-dose group was 25μg per day.Serum thyroid stimulating hormone(TSH)levels at different treatment times,complications and adverse drug reactions were compared between the two groups.Results Before treatment,there was no significant difference in TSH level between the two groups;after 2 weeks of treatment,the TSH levels of the two groups were lower than the previous time point,and the high-dose group was lower than the low-dose group,the differences were statistically significant(P<0.05).The incidence of complications in the high-dose group was lower than that in the low-dose group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion During the initial treatment,high-dose levothyroxine is effective in the treatment of SCH during pregnancy,reducing pregnancy complications and improving pregnancy outcomes.
