无抽搐电休克治疗仪联合药物治疗在难治性精神分裂症患者中的应用分析 被引量:1

Application analysis of modified electro-convulsive therapy instrument combined with drug therapy in patients with treatment-resistant schizophrenia

摘要 目的探究无抽搐电休克治疗(MECT)联合药物治疗在难治性精神分裂症(TRS)患者中的应用效果。方法选择2020年1月—2022年6月本院收治的TRS患者70例作为研究对象,采用随机数表法分为对照组与观察组两组,每组各35例。对照组行利培酮联合氯氮平药物治疗,观察组在对照组的基础上加以MECT联合治疗,两组均治疗12周。比较两组治疗前、12周后神经电生理指标情况,采用简易智能精神状况量表(MMSE)及蒙特利尔认知评估量表(MoCA)评价患者的认知能力,采用韦氏记忆量表(WMS)评价患者记忆力,并评价两组治疗的效果,统计治疗期间不良反应发生情况。结果两组的治疗有效率分别为91.43%和71.43%,观察组高于对照组(P<0.05)。12周后,观察组P300波幅、N2-P3波幅均高于对照组(P<0.05),其P300潜伏期、N2-P3潜伏期均低于对照组(P<0.05)。12周后,两组的MMSE及MoCA评分均较治疗前提高(P<0.05),且观察组上述评分均高于对照组(P<0.05)。治疗后观察组患者的逻辑思维、视觉再现、图形排列及文字配对等记忆能力指标评分均高于对照组(P<0.05)。两组的不良反应发生率分别为17.15%和14.29%,组间比较无统计学差异(P>0.05)。结论MECT联合药物治疗TRS,可明显提高患者的认知及记忆力,改善其神经电生理指标,较为安全有效。 Objective To explore the application effect of modified electro-convulsive therapy(MECT)combined with drug therapy in patients with treatment-resistant schizophrenia(TRS).Methods 70 patients with TRS who were admitted to the hospital from January 2020 to June 2022 were enrolled as study subjects.Using random number table method,the patients were divided into the control group and the observation group,with 35 cases in each group.The control group was treated with risperidone combined with clozapine.On this basis,the observation group was additionally given MECT.Both groups were given 12 weeks of treatment.The neuro-electrophysiological indicators were compared between the two groups before and after treatment.The Mini Mental State Examination(MMSE)scale and the Montreal Cognitive Assessment(MoCA)scale were used to evaluate cognitive ability,and the Wechsler Memory Scale(WMS)was used to evaluate memory.Treatment effects of the two groups were evaluated,and the occurrence of adverse reactions during treatment was counted.Results The effective rate of treatment in the observation group was 91.43%,which was higher than 71.43%in the control group(P<0.05).After treatment,the P300 amplitude and N2-P3 amplitude of the observation group were higher than those of the control group(P<0.05),and the P300 latency and N2-P3 latency were lower than those of the control group(P<0.05).After treatment,the MMSE scores and MoCA scores of the two groups were increased(P<0.05),and the above scores of the observation group were higher than those of the control group(P<0.05).After treatment,the memory scores in logical thinking,visual representation,graphic arrangement and text matching of the observation group were higher than those of the control group(P<0.05).The incidences of adverse reactions in the two groups were 17.15%and 14.29%,respectively,there was no significant difference between two groups(P>0.05).Conclusions MECT combined with drug therapy can significantly improve the cognition and memory of patients with TRS,and improve
