摘要
目的探究图像引导调强放射治疗(IGRT)联合特瑞普利单抗治疗局部晚期不能手术治疗的老年非小细胞肺癌(NSCLC)患者的效果。方法选取2019年1—12月泉州市光前医院肿瘤放疗科收治的60例NSCLC患者作为研究对象,随机分为研究组与对照组,每组30例。对照组使用IGRT技术,研究组在对照组基础上联合特瑞普利单抗治疗,比较两组近期疗效、毒副反应发生情况、生存时间。结果研究组临床有效率为86.67%,高于对照组的60.00%,差异有统计学意义(P<0.05);两组疾病控制率比较差异无统计学意义,两组WBC减少、血红蛋白减少、血小板减少、放射性肺炎、放射性食管炎、乏力、发热、心悸、心律失常、甲状腺功能减退发生情况比较差异无统计学意义,两组中位生存时间比较差异无统计学意义。结论IGRT联合特瑞普利单抗治疗局部晚期不能手术治疗的老年NSCLC患者的疗效显著,毒副反应患者可承受,值得临床推广应用。
Objective To investigate the efficacy of image-guided intensity modulated radiation therapy(IGRT)combined with tiriplizumab in the treatment of locally advanced and inoperable non-small cell lung cancer(NSCLC)in the elderly.Methods A total of 60 NSCLC patients admitted to the department of radiotherapy Quanzhou Guangqian Hospital from January to December 2019 were selected as the study subjects,and randomly divided into the study group and the control group,with 30 cases in each group.The control group was treated with IGRT technology,and the research group was treated with tiriplimab on the basis of the control group.The short-term efficacy,occurrence of toxic and side effects,and survival time were compared between the two groups.Results The clinical effective rate in the study group was 86.67%,which was higher than 60.00%in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in disease control rate between the two groups,there was no statistically significant difference in the WBC reduction,hemoglobin reduction,thrombocytopenia,radiation pneumonia,radiation esophagitis,fatigue,fever,palpitation,arrhythmia and hypothyroidism between the two groups,and there was no statistically significant difference in the median survival time between the two groups.Conclusion IGRT combined with terriprilizumab is effective in the treatment of locally advanced elderly NSCLC patients who cannot be treated with surgery,and the toxic and side effects can be tolerated in patients,which is worthy of clinical promotion and application.
