摘要
目的探究血管内皮生长因子(VEGF)预测ABO新生儿溶血病(HDN)患儿血管内皮损伤及其与溶血的发生和严重程度的关系。方法选取2020年4月至2020年11月黄岛区人民医院收治的50例ABOHDN患儿作为ABO-HDN组,另选取同期47例母婴血型匹配的健康新生儿作为对照组,比较两组同期血红蛋白(Hb)、间接胆红素(IBIL)VEGF水平,分析VEGF与溶血指标的相关性,采用多重回归分析ABO-HDN组VEGF水平与临床变量间的相关性。结果ABO-HDN组Hb水平低于对照组,IBIL、VEGF水平均高于对照组,差异有统计学意义(P<0.05)。且VEGF水平与IBIL水平呈正相关(r>0,P<0.05),而与Hb水平呈负相关(r<0,P<0.05)。多重回归分析结果显示,ABO-HDN组VEGF水平与Hb、IBIL水平呈现独立相关性(P<0.05)。结论VEGF在ABO-HDN患儿中的水平升高可反映患儿的内皮损伤,并有助于评估可能发生的严重血管功能障碍风险。
Objective To investigate the relationship between vascular endothelial growth factor(VEGF)and the occurrence and severity of hemolysis in ABO neonatal children with hemolysis.Methods 50 ABO neonatal hemolytic disease children admitted to Huangdao District People's Hospital from April 2020 to November 2020 were selected as the ABO-HDN group,and 47 healthy newborns with matching maternal and infant blood groups were selected as the control group.The detection data of hemoglobin(Hb)and indirect bilirubin(IBIL)were compared between the two groups during the same period.The concentration of vascular endothelial growth factor was quantitatively detected.Results The level of Hb in ABO-HDN group was lower than that in the control group,and the levels of IBIL and VEGF were higher than those in the control group,and the differences were statistically significant(P<0.05).VEGF levels were positively correlated with IBIL levels(r>0,P<0.05),but negatively correlated with Hb levels(r<0,P<0.05).Multiple regression analysis showed that VEGF levels were independently correlated with Hb and IBIL levels in ABO-HDN group(P<0.05).Conclusion Elevated levels of VEGF in ABO-HDN can be used to reflect endothelial injury and to assess the risk of severe vascular dysfunction.
