摘要
目的观察清热解毒散结方联合肝动脉化疗栓塞(TACE)术治疗原发性肝癌肝热血瘀型患者的临床疗效。方法将80例原发性肝癌肝热血瘀型患者按照随机数字表法分为2组,对照组40例予TACE术治疗,治疗组40例在对照组治疗基础上加用清热解毒散结方治疗,2组均连续治疗8周。比较2组临床疗效;比较2组治疗前后总胆红素(TBiL)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、糖类抗原199(CA199)、甲胎蛋白(AFP)水平变化;比较2组治疗前后症状评分变化。结果治疗组总有效率60.0%(24/40),对照组总有效率45.0%(18/40),治疗组临床疗效优于对照组(P<0.05)。2组治疗后TBiL、ALT、AST水平均较本组治疗前降低(P<0.05),治疗后治疗组TBiL、ALT、AST水平均低于对照组(P<0.05)。2组治疗后CA199、AFP水平均较本组治疗前降低(P<0.05),治疗后治疗组CA199、AFP水平均低于对照组(P<0.05)。2组治疗后恶心、肝区疼痛、乏力、纳差评分均较本组治疗前降低(P<0.05),治疗后治疗组恶心、肝区疼痛、乏力、纳差评分均低于对照组(P<0.05)。结论清热解毒散结方联合TACE术治疗原发性肝癌肝热血瘀型,能降低转氨酶水平,促进肝功能恢复,缓解临床症状,减轻术后副作用。
Objective To observe the clinical effect of Qingre Jiedu Sanjie Recipe combined with transarterial chemoembolization(TACE)on primary liver cancer(liver heat and blood stasis type).Methods Totally 80 cases with primary liver cancer(liver heat and blood stasis type)were randomly assigned into the control group(n=40)and the treatment group(n=40).All patients were managed by TACE,and patients in the treatment group were additionally given Qingre Jiedu Sanjie Recipe.The treatment was performed for 8 weeks,aiming to compare the clinical efficacy,and the levels of total bilirubin(TBiL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),carbohydrate antigen 199(CA199)and alpha fetoprotein(AFP)between the two groups before and after treatment.The syndrome scores were assessed.Results The total effective rate in the treatment group was better than that of the control group(60.0%[24/40]vs 45.0%[18/40],P<0.05).After treatment,the levels of TBiL,ALT and AST,as well as CA19-9,AFP in the two groups were significantly lower(all P<0.05),and the decrease in the treatment group was more significant(all P<0.05).The syndrome scores(fatigue,liver pain,nausea and anorexia)in the two groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).Conclusion For primary liver cancer(liver blood stasis type),Qingre Jiedu Sanjie Recipe combined with TACE can reduce the level of transaminase,promote the recovery of liver function,relieve clinical symptoms,reduce postoperative side effects.
