基层内镜医师pit pattern分型在线培训效果观察

Observation of the Effect of Online Training for Pit-pattern Classification for Primary Care Endoscopists

摘要 目的:观察新疆兵团基层内镜医师pit pattern分型的在线培训效果。方法:选取2021-2022年新疆兵团基层医院的内镜医师81名,将肠镜操作量≥3000例医师分为专家组39名,非专家组42名。对所有医师进行pit pattern分型在线培训和培训前、后测试。比较培训前后医师判断结直肠息肉pit pattern分型的准确率,并分析判断结果与正确结果的一致性。结果:对兵团基层内镜医师的pit pattern分型在线培训取得了满意的效果。培训前、后测试表现:(1)所有医师及专家组、非专家组医师培训后判断总体息肉pit pattern分型的准确率明显高于培训前(P<0.001),所有医师培训后判断肿瘤性、非肿瘤性息肉pit pattern分型的准确率高于培训前(P<0.001),且所有医师培训后判断息肉pit pattern分型的每一分型的准确率也高于培训前(P<0.001)。(2)相较于培训前,培训后所有医师及专家组、非专家组医师息肉pit pattern分型判断结果与正确结果的Kappa值有提高,且培训后所有医师息肉性质判断结果与正确结果的Kappa值也有提高。结论:在线培训的方式能够提升基层内镜医师使用pit pattern分型辨别结直肠息肉性质的能力。 Objective:To observe the effect of online training of pit pattern classification for primary endoscopists in Xinjiang Corps.Methods:A total of 81 endoscopists in primary hospitals of Xinjiang Corps from 2021 to 2022 were selected,and the endoscopists with more than 3000 colonoscopies were divided into an expert group of 39 and a non-expert group of 42.On-line training of pit pattern classification and pre-and post-training tests were conducted for all endoscopists.The accuracy of pit pattern classification of colorectal polyps by endoscopists before and after training was compared,and the consistency between the results and the correct results was analyzed.Results:Satisfactory results were achieved in the online training of pit pattern classification for primary endoscopic physicians in corps.Test performance before and after training:(1)The accuracy of judging the overall polyp pit pattern classification of all endoscopists,expert group and non-expert group endoscopists after training was significantly higher than that before training(P<0.001),the accuracy of judging the neoplastic and non-neoplastic polyps pit pattern classification of all endoscopists after training was higher than that before training(P<0.001),and the accuracy of all endoscopists in judging each type of polyp pit pattern after training was also higher than that before training(P<0.001).(2)Compared with before training,after training,the Kappa values of all endoscopists,expert group and non-expert group endoscopists on the judgment results and correct results of pit pattern classification of colorectal polyps were improved,and the Kappa values of all endoscopists on the judgment results and correct results of the nature of colorectal polyps were also improved.Conclusion:Online training can improve the ability of primary endo-scopists to identify the nature of colorectal polyps using pit pattern classification.
