摘要
目的探讨血清miR-195-5p和DRD1在妊娠期糖尿病合并高血压孕妇中的表达水平及意义。方法前瞻性选取2022年2月至2023年1月在本院定期产前检查并分娩的GDM患者269例,根据患者是否并发HDP,将患者分为GDM组(n=171)和GDM-HDP组(n=98),另选取正常健康的孕妇100例作为对照组。采用葡萄糖氧化酶法测定空腹血糖,qRT-PCR检测血清中miR-195-5p和DRD1相对表达水平,观察对照组、GDM组及GDM-HDP组的miR-195-5p、DRD1及空腹血糖水平;观察不同疾病严重程度血清miR-195-5p、DRD1及空腹血糖水平;Logistic回归模型分析GDM合并HDP的影响因素;Spearman秩相关分析血清miR-195-5p、DRD1和空腹血糖的相关性,采用受试者工作特征曲线(ROC)分析血清miR-195-5p、DRD1在GDM合并HDP中的诊断价值。结果与对照组相比,GDM组和GDM-HDP组中miR-195-5p的水平明显降低(P<0.001),DRD1及空腹血糖水平明显升高(P<0.001);与GDM组相比,GDM-HDP组中miR-195-5p的水平显著降低(P<0.001),DRD1及空腹血糖水平显著升高(P<0.001);GDM-HDP组患者血清中miR-195-5p水平明显高于轻度子痫前期、重度子痫前期患者,轻度子痫前期患者血清miR-195-5p水平显著高于重度子痫前期患者(P<0.001);GDM-HDP组患者血清中DRD1及空腹血糖水平明显低于轻度子痫前期、重度子痫前期患者,轻度子痫前期患者血清DRD1及空腹血糖水平显著低于重度子痫前期患者(P<0.001);miR-195-5p是GDM合并HDP发生的保护因素(P<0.001),而DRD1和空腹血糖是GDM合并HDP发生的危险因素(P<0.001);miR-195-5p与空腹血糖呈负相关(r=-0.276,P<0.001),DRD1与空腹血糖呈正相关(r=0.309,P<0.001);血清miR-195-5p诊断GDM合并HDP的AUC值为0.826,95%CI为0.769~0.883,血清DRD1诊断GDM合并HDP的AUC值为0.884,95%CI为0.837~0.929,miR-195-5p和DRD1联合诊断AUC值为0.952,95%CI为0.924~0.981,miR-195-5p和DRD1联合检测的AUC值、灵敏度、准确性明显高于miR-195-5p和DRD1单独检测,差异具有统计学意义(P<0.05
Objective To investigate the expression and significance of serum miR-195-5p and DRD1 in pregnant women with diabetes complicating hypertension.Methods Prospective selection of 269 patients with GDM who underwent regular prenatal examinations and delivered in our hospital from February,2022 to January,2023 was conducted for the current study.According to whether patients developed HDP,these patients were divided into GDM group(n=171)and GDM-HDP group(n=98),while another 100 healthy pregnant women were selected as the control group.Fasting blood glucose was measured by glucose oxidase method,and the relative expression levels of miR-195-5p and DRD1 in serum were measured by qRT-PCR.The levels of miR-195-5p,DRD1,and fasting blood glucose in the control group,GDM group,and GDM-HDP group were observed;The levels of serum miR-195-5p,DRD1,and fasting blood glucose in different disease severity levels were measured;Logistic regression model was used to analyze the influencing factors of GDM combined with HDP;Spearman rank correlation analysis was used to analyze the correlation between serum miR-195-5p,DRD1,and fasting blood glucose.The diagnostic value of serum miR-195-5p,DRD1 in GDM with HDP was analyzed using the receiver operating characteristic curve(ROC).Results Compared with the control group,the levels of miR-195-5p in GDM group and GDM-HDP group were significantly lower(P<0.001),while the levels of DRD1 and fasting blood glucose were significantly higher(P<0.001);Compared with the GDM group,the levels of miR-195-5p in the GDM-HDP group were significantly lower(P<0.001),while the levels of DRD1 and fasting blood glucose were significantly higher(P<0.001);The serum miR-195-5p levels in patients with GDM-HDP were significantly higher than those in patients with mild and severe preeclampsia,and the serum miR-195-5p levels in patients with mild and severe preeclampsia were significantly higher than those in patients with severe preeclampsia(P<0.001);The serum DRD1 and fasting blood glucose levels in patients with GD
