摘要
目的探讨度洛西汀联合认知行为干预(CBI)对首发重性抑郁障碍(FE-MDD)患者脂肪因子及临床结局的影响。方法选取2020年6月至2021年6月宜春市第三人民医院收治的60例FE-MDD患者作为研究对象,按照治疗方式的不同分为观察组(n=32)与对照组(n=28)。对照组采用度洛西汀治疗,观察组在对照组治疗基础上联合CBI治疗,比较两组治疗前后血清脂肪因子水平及临床结局。结果干预后,两组抵抗素水平均低于干预前,瘦素、脂联素水平均高于干预前,且观察组抵抗素水平低于对照组,瘦素、脂联素水平均高于对照组,差异有统计学意义(P<0.05)。干预后,两组抑郁症状评分均低于干预前,自杀倾向评分均高于干预前,且观察组抑郁症状评分低于对照组,自杀倾向评分高于对照组,差异有统计学意义(P<0.05)。结论度洛西汀联合CBI治疗FE-MDD可明显改善患者的脂肪因子水平和抑郁症状,控制其自杀倾向。
Objective To investigate the effects of duloxetine combined with cognitive behavioral intervention(CBI)on adipokine and clinical outcomes in patients with first-episode major depression disorder(FE-MDD).Methods A total of 60 FE-MDD patients admitted to the Third People's Hospital of Yichun from June 2020 to June 2021 were selected as the research subjects,and they were divided into the observation group(n=32)and the control group(n=28)according to different treatment methods.The control group was treated with duloxetine,and the observation group was treated with CBI on the basis of the control group.The serum adipokine levels before and after treatment and clinical outcomes were compared be-tween the two groups.Results After intervention,resistin levels of the two groups were lower than before intervention,while leptin and adiponectin levels were higher than before intervention,and resistin levels in the observation group were lower than the control group,while leptin and adiponec-tin levels were higher than those in the control group,the differences were statistically significant(P<0.05).After intervention,the score of depres-sive symptoms of the two groups was lower than before intervention,and the score of suicidal tendency was higher than before intervention,and the score of depressive symptoms in the observation group was lower than that in the control group,and the score of suicidal tendency was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion Duloxetine combined with CBI can significantly im-prove the level of adipokine and depressive symptoms in FE-MDD patients,and control their suicidal tendency.
