下肢开放性骨折并皮肤缺损患者应用外固定支架联合克氏针及VSD技术的近中期疗效分析

摘要目的分析外固定支架、克氏针和负压封闭引流技术(vacuum scaling drainage,VSD)联合应用于下肢开放性骨折并皮肤缺损患者应用的近中期的疗效。方法将2019年7月—2022年6月潮州市中心医院收治的60例下肢开放性骨折且合并皮肤缺损患者作为研究对象,采用中心随机法将其分为实验组(n=30)以及对照组(n=30)。对照组采用外固定支架治疗,实验组采用外固定支架联合克氏针及VSD技术治疗。对比两组术后指标、治疗12个月、18个月、24个月总有效率以及并发症发生率。结果实验组下肢肿胀消失时间、下肢麻木消退、足背动脉搏动恢复、创面愈合时间均短于对照组(P<0.05);换药次数、VAS疼痛评分均小于对照组(P<0.05);治疗3个月、6个月、12个月、18个月、24个月后实验组总有效率均高于对照组,且治疗12个月、18个月、24个月实验组总有效率与对照组总有效率差异有统计学意义(P<0.05);实验组的并发症发生率小于对照组(P<0.05)。结论采用外固定支架联合克氏针及VSD技术治疗下肢开放性骨折并皮肤缺损患者的具有较好的近中期疗效,可有效改善患者术后临床指标、减少术后疼痛与并发症,提高治疗有效性。 Objective To analyze short-term and medium-term effect of combination of external fixation,Kirschner wires,and vacuum scaling drainage(VSD)technology for patients with open lower limb fractures and skin defects of.Methods The paper chose 60 patients with open lower limb fractures and skin defects admitted to Chaozhou Central Hospital from July 2019 to June 2022 as study subjects,and divided them into experimental group(n=30)and control group(n=30)randomly with central randomization method.Control group was treated with external fixation,while experimental group was treated with external fixation combined with Kirschner wire and VSD technology.Postoperative indicators,total effective rates at 12,18,and 24 months of treatment,and incidence of complications were compared between two groups.Results Disappearance time of lower limb swelling,disappearance of lower limb numbness,recovery of dorsal foot artery pulsation,and wound healing time in experimental group was shorter than control group(P<0.05).Number of dressing changes and VAS pain scores in control group were lower than control group(P<0.05).After 3,6,12,18,and 24 months of treatment,total effective rate of experimental group was higher than control group,there was statistically significant difference in total effective rate between experimental group and control group at 12,18,and 24 months of treatment(P<0.05).in cidence of complications in experimental group was 3.33%,lower than 20.00%in control group(P<0.05).Conclusion External fixation combined with Kirschner wires and VSD technology can achieve good short-term and medium-term effect for patients with open lower limb fractures and skin defects,and can improve postoperative clinical indicators,reduce postoperative pain and complications,and enhance treatment effectiveness effectively.
