无创呼吸机在重症小儿肺炎合并急性呼吸衰竭治疗中的临床效果

Clinical Effect of Noninvasive Ventilator in the Treatment of Severe Pediatric Pneumonia Complicated with Acute Respiratory Failure

摘要 目的:探讨无创呼吸机在重症小儿肺炎合并急性呼吸衰竭治疗中的临床效果。方法:选取2020年1月—2023年6月常州市儿童医院收治的重症肺炎合并急性呼吸衰竭患儿72例为研究对象,按照随机编号分组方式分为对照组与观察组,各36例。对照组采用鼻导管吸氧治疗,观察组采用无创呼吸机治疗。比较两组患儿动脉血气指标与临床疗效。结果:治疗前,两组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))和血氧饱和度(SpO_(2))水平比较,差异无统计学意义(P>0.05);治疗后,两组PaO_(2)和SpO_(2)水平均上升,PaCO_(2)水平均下降,观察组PaO_(2)和SpO_(2)水平高于对照组,PaCO_(2)水平低于对照组,差异有统计学意义(P<0.05)。观察组临床疗效有效率高于对照组,差异有统计学意义(P=0.047)。结论:无创呼吸机可以提高重症小儿肺炎合并急性呼吸衰竭的临床疗效,改善动脉血气指标。 Objective:To investigate the clinical effect of noninvasive ventilator in the treatment of severe pediatric pneumonia complicated with acute respiratory failure.Methods:A total of seventy-two children with severe pneumonia complicated with acute respiratory failure admitted to Changzhou Children's Hospital from January 2020 to June 2023 were selected as the study objects,and were divided into control group and observation group according to random numbering,with thirty-six cases in each group.The control group was treated with nasal catheter oxygen inhalation,and the observation group was treated with noninvasive ventilator.The indexes of arterial blood gas and clinical efficacy were compared between the two groups.Results:Before treatment,there was no significant difference in the level of arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))and blood oxygen saturation(SpO_(2))between the two groups(P>0.05);after treatment,PaO_(2) and SpO_(2) levels increased and PaCO_(2) levels decreased in both groups,PaO_(2) and SpO_(2) levels in the observation group were higher than those in the control group,and PaCO_(2) level was lower than that in the control group,with statistical significance(P<0.05).The effective rate of clinical efficacy of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.047).Conclusion:Noninvasive ventilator can improve the clinical effect of severe pediatric pneumonia complicated with acute respiratory failure and improve arterial blood gas index.
