产妇肥胖对母乳成分、新生儿代谢及营养状态的影响

摘要 目的:探究产妇肥胖对母乳成分、新生儿代谢及营养状态的影响。方法:选取定期产检并分娩的产妇320例为研究对象,根据体重增长情况分为A组(正常体重产妇)97例、B组151(超重产妇)例、C组(肥胖产妇)72例。观察、对比三组产妇的母乳成分、新生儿血糖、血脂四项及营养状态指标水平。结果:三组产妇初乳、过渡乳、成熟乳中母乳蛋白质浓度比较差异均无统计学意义(P>0.05);B组、C组的初乳中乳糖浓度均高于A组,差异有统计学意义(P<0.05);三组产妇过渡乳和成熟乳中母乳乳糖浓度比较差异均无统计学意义(P>0.05);B组、C组的初乳中脂肪浓度比较差异无统计学意义(P>0.05),但B组、C组的过渡乳中脂肪浓度低于A组,B组、C组的成熟乳中脂肪浓度高于A组,差异有统计学意义(P<0.05)。新生儿的血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL)水平比较,B组、C组均高于A组;高密度脂蛋白(HDL)、血糖水平比较,B组与C组均低于A组,差异均有统计学意义(P<0.05),但B组、C组新生儿血糖、血脂四项比较差异无统计学意义(P>0.05)。三组新生儿在6月龄的Kaup指数较出生时显著升高,其中B组、C组出生时与6月龄的Kaup指数均高于A组,差异有统计学意义(P<0.05),B组、C组比较差异无统计学意义(P>0.05)。结论:产妇超重或肥胖均可影响不同时期的母乳成分,并可在一定程度上影响新生儿糖脂代谢及营养状态。 Objective To explore the impact of maternal obesity on breast milk composition,neonatal metabolism,and nutritional status.Method 320 postpartum women who underwent regular prenatal examinations and delivered at our hospital were selected as research object.They were divided into group A 97 cases(normal weight postpartum women),group B 151 cases(overweight postpartum women),and group C 72 cases(obese postpartum women)based on their weight gain.Observe and compare the levels of breast milk composition,newborn blood glucose,blood lipids,and nutritional status indicators.Results There was no significant difference in the protein concentration of breast milk in colostrum,transitional milk,and mature milk among the three groups of postpartum women(P>0.05);The lactose concentrations in colostrum of group B and group C were higher than those of group A(P<0.05);There was no significant difference in lactose concentration in breast milk between transition milk and mature milk among the three groups of postpartum women(P>0.05);There was no significant difference in fat concentration in colostrum between group B and group C(P>0.05),but the fat concentration in transitional milk of group B and group C were lower than that of group A,while the fat concentration in mature milk of group B and group C were higher than that of group A(P<0.05).Compared with the levels of TC,TG,and LDL in newborns,the levels of group B and group C were higher than those in group A;Comparison of HDL and blood glucose levels showed that group B and group C were lower than those in group A(P<0.05),but there was no significant difference in neonatal blood glucose and lipid levels between group B and group C(P>0.05).The Kaup index of the three groups of newborns at 6 months of age was significantly higher than that at birth.Among them,the Kaup index of the group B and C at birth and 6 months of age was higher than that of the group A(P<0.05),and there was no significant difference between the group B and C(P>0.05).Conclusion Maternal overweight or obesit
