摘要
目的:观察头穴丛刺联合Bobath技术治疗脑卒中后上肢肌张力增高的临床疗效。方法:选取96例脑卒中后上肢肌张力增高患者,按照随机数字表法分为对照组与观察组,每组48例。治疗过程中,2组分别剔除2例,最终纳入研究各46例。对照组给予Bobath技术治疗,观察组给予头穴丛刺联合Bobath技术治疗,2组均治疗4周。比较2组临床疗效,治疗前后上肢运动功能及生活活动能力。结果:治疗后,观察组总有效率91.30%,高于对照组73.91%(P<0.05)。治疗后,2组Fugl-Meyer运动功能量表上肢部分(FMA-UE)评分均较治疗前升高(P<0.05),上肢功能评定表(DASH)评分均较治疗前降低(P<0.05);观察组FMA-UE评分高于对照组(P<0.05),DASH评分低于对照组(P<0.05)。治疗后,2组改良Barthel指数评定量表(MBI)评分均较治疗前升高(P<0.05),观察组MBI评分高于对照组(P<0.05)。结论:头穴丛刺联合Bobath技术能有效治疗脑卒中后上肢肌张力增高,改善患者上肢运动功能及生活活动能力。
Objective:To observe the clinical effect of the therapy of scalp point cluster-needling combined with Bobath technique on increased muscle tone of upper limbs after stroke.Methods:A total of96 cases of patients with increased muscle tone of upper limbs after stroke were selected and divided into the control group and the observation group according to the random number table method,with 48 cases in each group.During treatment,2 cases dropped off in both groups,and finally 46 cases were included in each group.The control group was treated with Bobath technique,and the observation group was treated with scalp point cluster-needling combined with Bobath technique.Both groups were treated for 4 weeks.The motor function of upper limbs and activities of daily living before and after treatment as well as the clinical effects were compared between the two groups.Results:After treatment,the total effective rate was 91.30% in the observation group,higher than that of 73.91% in the control group(P<0.05).After treatment,the scores of Fugl-Meyer Assessment Upper Extremity Scale(FMA-UE) in both groups were increased when compared with those before treatment(P<0.05),and the scores of Disabilities of the Arm,Shoulder and Hand(DASH) were decreased when compared with those before treatment(P<0.05);the FMA-UE score in the observation group was higher than that in the control group(P<0.05),and the DASH score was lower than that in the control group(P<0.05).After treatment,the scores of Modified Barthel Index(MBI) in the two groups were increased when compared with those before treatment(P<0.05),and the MBI score in the observation group was higher than that in the control group(P<0.05).Conclusion:The therapy of scalp point cluster-needling combined with Bobath technique can effectively treat the increased muscle tone of upper limbs after stroke and improve the motor function of upper limbs and activities of daily living.
