摘要
目的 研究糖尿病并发脓毒症患者应用二甲双胍的临床治疗效果。方法 选择2020年10月至2023年10月在我院接受治疗的60例糖尿病患者作为研究对象,依据不同治疗方式分成常规组和分析组,常规组采用抗生素控制感染、胰岛素纠正血糖、纠正酸中毒等药物治疗,研究组基于常规治疗联合二甲双胍治疗。对比2组患者病死率、治疗效果、乳酸水平、血清炎性因子、尿素氮(BUN)、血清肌酐(Scr)值等变化。采用SPSS19.0软件包进行统计分析,计数资料组间比较运用χ^(2)检验,计量资料组间比较运用t检验。结果研究组治疗病死率(20.0%)显著优于常规组(46.7%),差异有统计学意义(χ^(2)=4.800,P<0.05);研究组治疗有效率达(96.7%)高于常规组(66.7%),差异有统计学意义(χ^(2)=13.293,P<0.05);治疗后研究组炎性因子降钙素原(PCT)、白细胞介素(IL)-6、C反应蛋白(CRP)及BUN、Scr值,均低于常规组,差异有统计学意义(P<0.05)。结论二甲双胍可能通过在不同的器官中抑制促炎细胞因子的产生,改善糖尿病并发脓毒症临床症状和脓毒症的肾脏等器官损伤,降低病死率。
Objective To study the clinical therapeutic effect of metformin in patients with diabetes complicated with sepsis.Methods Sixty patients with diabetes complicated with sepsis who were treated in our hospital were selected as the study subjects,and the study was carried out from October 2020 to October 2023.According to different treatment methods,they were divided into the conventional group and the analysis group.The former was treated with antibiotics to control infection,insulin to correct blood sugar,and acid poisoning,while the latter was treated with metformin based on this.The mortality,treatment effect,lactic acid level serum inflammatory factors,urea nitrogen(BUN),serum creatinine(Scr) values,etc.were analyzed.Statistical analysis was conducted using SPSS 19.0 software package,and inter group comparison of counting data using χ^(2)t-ests,the t-test was used to compare the measurement data between groups.Results showed that the mortality rate of the treatment group(20.0%) was significantly higher than that of the control group(46.7%),and the difference was statistically significant(χ^(2)=4.800,P<0.05),the effective rate of treatment in the analysis group was 96.7%,which was higher than that in the control group(66.7%),and the difference was statistically significant (χ^(2)=13.293,P<0.05);After treatment,the inflammatory factors PCT,IL-6,CRP,BUN,and Scr values in the analysis group were lower than those in the control group,and the differences were statistically significant( P <0.05).Conclusion Metformin can inhibit the production of pro-inflammatory cytokines such as IL-6 in different organs,reduce mortality,improve clinical symptoms and kidney damage in sepsis,and truly benefit patients.
