摘要
目的 观察二甲双胍联合格列吡嗪治疗初诊糖尿病(DM)患者的临床疗效及其对微小RNA(mi RNA)表达水平的影响。方法 回顾性选取2019年10月-2021年9月瑞金市中医院收治的66例初诊DM患者作为研究对象,根据治疗方法分为对照组33例和观察组33例。对照组采用格列吡嗪治疗,观察组在对照组基础上予以二甲双胍治疗。2组均连续治疗6个月。比较2组治疗前及治疗6个月后血糖[糖化血红蛋白(Hb A)、餐后2 h血糖(2 h PG)及空腹血糖(FPG)]、血脂指标[三酰甘油(TG)、总胆固醇(TC)]、mi RNA表达水平、胰岛功能指标[空腹胰岛素(FINS)、餐后2 h胰岛素(2 h INS)、胰岛素抵抗指数(HOMA-IR)]及不良反应发生率。结果 2组治疗前Hb A、2 h PG、FPG、TG、TC水平及治疗后TG、TC水平比较,差异无统计学意义(P>0.05);治疗6个月后,2组Hb A、2 h PG、FPG、TG、TC水平低于治疗前,且观察组Hb A、2 h PG、FPG水平低于对照组(P <0.05)。治疗6个月后,2组mi R-29a、mi R-503表达水平低于治疗前,mi R-126表达水平高于治疗前,且观察组mi R-29a、mi R-503表达水平低于对照组,mi R-126表达水平高于对照组(P <0.01)。治疗6个月后,2组FINS、2 h INS水平高于治疗前,HOMA-IR低于治疗前,且观察组FINS、2 h INS水平高于对照组,HOMA-IR低于对照组(P <0.01)。2组治疗期间均未出现严重不良发应。结论 二甲双胍联合格列吡嗪治疗初诊DM患者的效果明显,可更有效地降低患者血糖水平,改善胰岛功能,调控mi RNA表达水平,且具有较高的安全性。
Objective To investigate the clinical effect of metformin combined with glipizide in the treatment of newly diagnosed diabetes mellitus( DM) patients and the impact on mi RNA expression level. Methods A total of 66 newly diagnosed DM patients were retrospectively selected in Ruijin Hospital of Traditional Chinese Medicine from October 2019 to September 2021,and thy were divided into control group( 33 cases) and observation group( 33 cases) according to treatment methods. The control group was treated with glipizide,and the observation group was treated with metformin based on control group. Both groups were treated continuously for 6 months. Blood glucose indexes( Hb A,2 h PG,FPG),blood lipid indexes( TG,TC),mi RNA expression levels and islet function indexes( FINS,2 h INS,HOMA-IR) before treatent and 6 months after treatment,and the incidence of adverse reactions were compared between the two groups. Results There was no significant difference of levels of Hb A,2 h PG,FPG,TG,TC before treatment and levels of TG,TC at 6 months after treatment between the two groups( P > 0. 05);at 6 months after treatment,levels of Hb A,2 h PG,FPG,TG,TC in the two groups were lower than those before treatment,and levels of Hb A,2 h PG,FPG in the observation were lower than those in control group( P < 0. 05). At 6 months after treatment,expression levels of mi R-29a,mi R-503 in the two groups were lower than those before treatment,expression level of mi R-126 was higher than that before treatment,and expression levels of mi R-29a,mi R-503 in the observation group were lower than those in control group,expression level of mi R-126 was higher than that in control group( P < 0. 01).
