HPV感染对慢性宫颈炎患者阴道微生态及miR-21、COX-2和炎性因子表达的影响 被引量:2

Influence of HPV infection on vaginal microflora,expressions of miR-21,COX-2 and inflammatory factors in patients with chronic cervicitis

摘要 目的研究人乳头瘤病毒(HPV)感染对慢性宫颈炎患者阴道微生态及微小核糖核酸(miR-21)、环氧合酶-2(COX-2)和炎性因子表达的影响。方法选取2019年6月—2023年2月天津市第五中心医院收治的204例HPV感染慢性宫颈炎患者和210例HPV阴性慢性宫颈炎患者分别作为HPV阳性组、HPV阴性组,比较两组阴道微生态状况及miR-21、COX-2、炎性因子水平。结果HPV阳性组阴道支原体、滴虫、霉菌感染患者占比及细菌性阴道炎、阴道微生态失调患者占比高于HPV阴性组(P<0.05);HPV阳性组miR-21、COX-2、白细胞介素(IL)-4、肿瘤坏死因子-α(TNF-α)水平高于HPV阴性组,IL-2、γ干扰素(IFN-γ)水平低于HPV阴性组(P<0.05);高危HPV感染患者miR-21、COX-2、IL-4水平高于低危感染患者,IL-2、IFN-γ水平低于低危感染患者(P<0.05);阴道微生态失调患者miR-21、COX-2、IL-4水平高于阴道微生态正常患者,IL-2、IFN-γ水平低于阴道微生态正常患者(P<0.05)。结论HPV感染可引起慢性宫颈炎患者阴道微生态紊乱,同时影响miR-21、COX-2、炎性因子水平,且HPV感染慢性宫颈炎患者高危型感染及阴道微生态失调也会引起miR-21、COX-2、炎性因子水平异常变化。 OBJECTIVE To observe the influence of human papillomavirus(HPV)infection on vaginal microflora,expressions of micro ribonucleic acid(miR-21),cyclooxygenase-2(COX-2)and inflammatory factors in the pa-tients with chronic cervicitis.METHODS Totally 204 chronic cervicitis with HPV infection and 210 chronic cervi-citis patients without HPV infection who were treated in the Tianjin Fifth Central Hospital from Jun 2019 to Feb 2023 were enrolled in the study and were respectively assigned as the HPV-positive group and the HPV-negative group.The vaginal microflora and levels of miR-21,COX-2 and inflammatory factors were observed and com-pared between the two groups.RESULTS The proportions of patients with Mycoplasma vaginalis,trichomoniasis and mycotic infections were in the HPV-positive group than in the HPV-negative group(P<0.05).The levels of miR-21,COX-2,interleukin(IL)-4 and tumor necrosis factor-α(TNF-α)of the HPV-positive group were higher than those of the HPV-negative group;while the levels of IL-2 and interferon-γ(IFN-γ)of the HPV-positive group were lower than those of the HPV-negative group(P<0.05).The levels of miR-21,COX-2 and IL-4 of the patients with high-risk HPV infection were higher than those of the patients with low-risk HPV infection,while the levels of IL-2 and IFN-γof the patients with high-risk HPV infection were lower than those of the patients with low-risk HPV infection(P<0.05).The levels of miR-21,COX-2 and IL-4 of the patients with vaginal dys-bacteriosis were higher than those of the patients with normal vaginal microflora;while the levels of IL-2 and IFN-y of the patients with vaginal dysbacteriosis were lower than those of the patients with normal vaginal microflora(P<0.05).CONCLUSION The HPV infection may result in the disorder of vaginal microflora in the patients with chronic cervicitis and affect the levels of miR-21,COX-2 and inflammatory factors.The high-risk HPV in-fection and vaginal dysbacteriosis may also lead to the abnormal changes of miR-21.COX-2 and inflammatory fac-tors
